摘 要:目的:探讨自拟益气除湿活血汤对肥胖大鼠骨骼肌炎性反应改善作用及其机制。方法:高脂饲料喂养建立肥胖大鼠模型,将造模成功的80只SD大鼠随机分成4组:模型组、自拟益气除湿活血汤低(3.75 g·kg^(-1))、中(7.5 g·kg^(-1))、高(15 g·kg^(-1))剂量组,每组20只,另取20只SD大鼠喂以普通饲料作为正常对照组。自拟益气除湿活血汤低、中、高剂量组分别灌胃相应药物,1次/d,持续8周,正常对照组和模型组灌胃等体积的生理盐水。给药周期结束后,测定大鼠血液淋巴细胞、嗜酸性粒细胞、中性粒细胞水平及血清白细胞介素-2(IL-2)、白细胞介素-4(IL-4)、肿瘤坏死因子-α(TNF-α)水平,观察大鼠骨骼肌组织病理结构,实时荧光定量PCR及蛋白免疫印迹法测定人肌萎缩蛋白Fbox-1(Atrogin-1)、肌肉特异性环指蛋白1(MuRF1)mRNA和蛋白水平。结果:正常对照组骨骼肌结构正常,肌纤维排列整齐;模型组骨骼肌梗死区可见炎症细胞浸润,骨骼肌纤维断裂,排列紊乱,可见明显瘢痕组织;自拟益气除湿活血汤低、中、高剂量组骨骼肌纤维排列较为整齐,瘢痕组织较少,炎症细胞浸润减少。与正常对照组比较,模型组大鼠血液淋巴细胞、嗜酸性粒细胞、中性粒细胞、血清IL-2、IL-4、TNF-α、骨骼肌组织Atrogin-1、MuRF1 mRNA和蛋白水平显著升高(P<0.05);与模型组比较,自拟益气除湿活血汤低、中、高剂量组大鼠血液淋巴细胞、嗜酸性粒细胞、中性粒细胞、血清IL-2、IL-4、TNF-α、骨骼肌组织Atrogin-1、MuRF1 mRNA和蛋白水平显著降低(P<0.05),且呈剂量依赖性(P<0.05)。结论:自拟益气除湿活血汤对肥胖大鼠骨骼肌炎性反应具有明显改善作用,其机制可能与自拟益气除湿活血汤抑制Atrogin-1和MuRF1的表达进而抑制炎性信号通路有关。Objective:To investigate the effect and mechanism of self-made Yiqi Chushi Huoxue decoction on the inflammatory reactions of skeletal muscle in obese rats.Methods:Obese rat model was established by high fat feeding,and 80 SD rats successfully modeled were randomly divided into 4 groups:model group,self-made Yiqi Chushi Huoxue decoction low(3.75 g·kg^(-1)),medium(7.5 g·kg^(-1))and high(15 g·kg^(-1))dose groups with 20 ones in each group,another 20 SD rats were fed with regular chow as normal control group.The self-made Yiqi Chushi Huoxue decoction low,medium and high dose groups were given corresponding drug by gavage,once a day for 8 weeks,and the normal control group and the model group were given the same volume of normal saline.At the end of administration cycle,the levels of blood lymphocytes,eosinophil,neutrophils,serum interleukin-2(IL-2),interleukin-2(IL-4)and tumor necrosis factor-α(TNF-α)were measured,the pathological structure of skeletal muscle was observed,and the m RNA and protein levels of Atrogin-1 and MuRF1 in skeletal muscle were determined by real-time PCR and Western blot.Results:The skeletal muscle structure of the normal control group was normal,and the muscle fibers were arranged orderly;the skeletal muscle in the model group showed inflammatory cell infiltration,broken skeletal muscle fibers,disordered arrangement and obvious scar tissue;in the self-made Yiqi Chushi Huoxue decoction low,medium and high dose groups,the skeletal muscle fibers were arranged orderly with less scar tissue and less inflammatory cell infiltration.Compared with those in the normal control group,the levels of blood lymphocytes,eosinophils,neutrophils,serum IL-2,IL-4 and TNF-α,skeletal muscle tissue Atrogin-1,MuRF1 m RNA and protein levels in the model group were increased significantly(P<0.05);compared with those in the model group,the levels of blood lymphocytes,eosinophils,neutrophils,serum IL-2,IL-4 and TNF-α,skeletal muscle tissue Atrogin-1,MuRF1 m RNA and protein levels in the selfmade Yiqi Chushi Huoxue
