重楼生化汤中药配方颗粒预防剖宫产术后恶露不净效果观察

摘　　要：目的:观察重楼生化汤中药配方颗粒预防剖宫产术后恶露不净的临床有效性及安全性。方法:子宫下段剖宫产产妇400例随机分为A、B、C、D 4组,每组100例,分别于剖宫产术后6 h起给予重楼生化汤中药配方颗粒、重楼生化汤、经典生化汤和安慰剂,连用15 d。观察比较各组患者的子宫复旧(住院期间耻骨上子宫长度、术后B超监测子宫三径之和与子宫切口I类愈合率)、恶露(血性恶露量及恶露持续时间)、术后肛门排气时间,以及血常规与血清CRP、TNF-α、IL-1β水平;评价各组患者满意度。结果:A、B两组子宫复旧、恶露、术后肛门排气时间,以及血常规与血清CRP、TNF-α、IL-1β水平等各项指标均显著优于C、D两组(P<0.05);而A、B两组间各项指标比较,差异均无统计学意义(P>0.05)。术后42天产妇满意度评分4组间有显著差异,A、B、C、D组评分依次降低(P<0.01)。结论:剖宫产术后应用重楼生化汤中药配方颗粒预防恶露不净效果明显,与其传统中药汤剂疗效相当,且患者满意度更高。Objective:To explore the clinical effectiveness and safety of Chonglou Shenghua decoction concentrate-granules in preventing lochia after cesarean section.Methods:From January 2017 to December 2019,400 cases of cesarean section were performed and divided into 4 groups according to the random number table method with 100 cases in each group:Chonglou Shenghua decoction concentrate-granules group(group A),Chonglou Shenghua decoction group(group B),classic Shenghuatang group(group C)and placebo group(group D).Each group was administrated the corresponding drug since the 6 th hour after surgery,and the treatment course was 15 days.Uterine rejuvenation including the height of uterine fundus during hospitalization,the sum of three diameters of uterus by B-ultrasound monitoring and the class I healing of uterine incision,lochia including the amount and duration of bloody lochia,anal exhaust time after surgery,blood routine and serum levels of CRP,TNF-αand IL-1βwere observed and compared among the 4 groups.Patients’satisfaction was also investigated.Results:The above indicators of groups A and B were all better than those of groups C and D(P<0.05),and there were no significant differences between group A and group B(P>0.05).The difference in parturient women’s satisfaction at the 42 nd day after operation was significant among the 4 groups(P<0.01)with the descending order of group A,B,C and D.Conclusion:The application of Chonglou Shenghua decoction concentrate-granules after cesarean section can obviously prevent the occurrence of lochia,and the drug has good safety and high satisfaction of patients.
