化痰活血方治疗冠心病稳定型心绞痛(痰阻血瘀证)的疗效及对sdLDL-C、hs-CRP的影响

摘　　要：目的观察化痰活血方对冠心病稳定型心绞痛(痰阻血瘀证)的临床疗效及对小而密低密度脂蛋白胆固醇(sdLDL-C)、超敏C反应蛋白(hs-CRP)的影响并探讨其作用机制。方法80例患者按随机数字表法分为对照组与治疗组各40例。对照组予调脂、扩血管、抗血小板聚集等西医常规治疗,治疗组在常规治疗基础上予化痰活血方中药复方治疗,治疗时间为4周。观察比较两组患者心绞痛发作次数、发作时间、硝酸甘油治疗量、中医证候积分、心电图疗效以及血清sdLDL-C、hs-CRP水平。结果治疗后治疗组总有效率95.00%高于对照组的77.50%(P<0.05)。两组治疗1、2、4周后心绞痛发作次数、发作时间、硝酸甘油治疗量与治疗前比较均降低(均P<0.05),且治疗组治疗1、2、4周后心绞痛发作次数、发作时间、硝酸甘油治疗量均低于对照组(均P<0.05)。两组治疗1、2、4周后两组中医证候积分与治疗前比较均降低(均P<0.05),且治疗组治疗1、2、4周后中医证候积分均低于对照组(均P<0.05)。两组治疗后血清sdLDL-C、hs-CRP水平与治疗前比较,均降低(均P<0.05),且治疗后治疗组血清sdLDL-C、hs-CRP水平均低于对照组(均P<0.05)。治疗期间两组均无明显不良反应,治疗后两组各项肝肾功能实验室指标均正常,无并发症发生。结论化痰活血方治疗冠心病稳定型心绞痛(痰阻血瘀证)临床疗效明显,既可改善心绞痛症状及中医证候、提高患者生活质量,又能降低血浆sdLDL-C、hs-CRP水平。Objective:To observe the clinical efficacy of Hatan Huoxue Recipe on stable angina pectoris of coro⁃nary heart disease(phlegm and blood stasis type)and its effect on sdLDL-C,hs-CRP.Methods:A total of 80 pa⁃tients were divided into the control group and treatment group by random number table method,40 cases in each group.The control group was treated with conventional western medicine such as lipid regulation,vasodilation,and anti-platelet aggregation,and the treatment group was treated with traditional Chinese medicine Hatan Huoxue Rec⁃ipeq on the basis of conventional treatment.The treatment period was 4 weeks.The frequency and duration of angi⁃na pectoris attack,the amount of nitroglycerin treatment,TCM syndrome score,electrocardiogram efficacy,and se⁃rum SDLDL-C and hs-CRP levels were observed and compared between two groups.Results:After treatment,the total effective rate in the treatment group was 95.00%,which was higher than 77.50%in the control group(P<0.05).After 1,2,and 4 weeks of treatment,the frequency and duration of angina pectoris attack and the amount of nitroglycerin treatment were all reduced compared with those before treatment(all P<0.05),and levels in the treat⁃ment group were lower than those in the control group(all P<0.05).After 1,2,and 4 weeks of treatment,the scores of TCM syndromes in the two groups were lower than those before treatment(all P<0.05),and the scores in the treatment group were lower than those of the control group(all P<0.05).After treatment the sdLDL-C and hsCRP levels in the two groups were lower than before treatment(all P<0.05),and the levels in the treatment group were lower than those in the control group(all P<0.05).During the treatment,there were no obvious adverse reac⁃tions in the two groups.After treatment,the liver and kidney function laboratory indexes of the two groups were normal,and no complications occurred.Conclusion:Hatan Huoxue Recipe has good efficacy on patients with coro⁃nary heart disease with stable angina pectoris(phlegm and blood
