派瑞松联合制霉菌素治疗难治性真菌性外耳道炎患者的疗效

摘　　要：目的探讨派瑞松联合制霉菌素治疗难治性真菌性外耳道炎患者的疗效。方法选择我院2016年11月至2019年2月收治的50例难治性真菌性外耳道炎患者。随机分组,常规组实施冲洗外耳道治疗,综合组实施冲洗外耳道+派瑞松联合制霉菌素治疗。比较两组的临床疗效、瘙痒症状消失时间、红肿消失时间、耳闷消失时间、渗出消除时间,治疗前后患者的症状积分、炎性因子以及不良反应发生情况。结果综合组治疗总有效率高于常规组(P<0.05)。综合组瘙痒症状消失时间、红肿消失时间、耳闷消失时间、渗出消除时间均短于常规组(P<0.05)。治疗前,两组症状积分、TNF-α、Hs-CRP、IL-6比较,差异无统计学意义(P>0.05);治疗后,综合组症状积分、TNF-α、Hs-CRP、IL-6均低于常规组,差异有统计学意义(P<0.05)。综合组不良反应发生率为8.00%,常规组为4.00%,差异无统计学意义(P>0.05)。结论冲洗外耳道+派瑞松联合制霉菌素治疗难治性真菌性外耳道炎患者的效果确切,有助于改善患者的临床症状和炎性因子,促进患者快速康复。Objective To investigate the efficacy of pevisone combined with nystatin in the treatment of refractory fungal otitis externa.Methods Fifty patients with refractory fungal otitis externa from November 2016 to February 2019 were enrolled in our hospital.Randomized,the routine group was treated with flushing external auditory canal,and the comprehensive intervention group was treated with flushing external auditory canal+parisone combined with nystatin.The clinical efficacy,disappearance time of pruritus symptoms,disappearance time of redness and swelling,disappearance time of ear stuffiness,disappearance time of exudation were compared between the two groups.Results The total effective rate of treatment in the comprehensive group was higher than that in the conventional group(P<0.05).The disappearance time of pruritus symptoms,the disappearance time of redness and swelling,the disappearance time of ear stuffiness,and the elimination time of exudation in the comprehensive group were shorter than those in the conventional group(P<0.05).Before treatment,there was no significant difference in symptom score,TNF-α,Hs-CRP,IL-6 between the two groups(P>0.05);after treatment,the symptom score,TNF-α,Hs-CRP,IL-6 were lower than the conventional group,the difference was statistically significant(P<0.05).The adverse reaction rate was 8.00%in the comprehensive group and 4.00%in the routine group,the difference was not statistically significant(P>0.05).Conclusion The effect of flushing the external auditory canal+parvisone combined with nystatin in the treatment of patients with refractory fungal otitis externa is definite,which helps to improve the clinical symptoms and inflammatory factors of the patients and promote the rapid recovery of the patients.
