老年高血压患者的动态脉压与冠心病并发症、颈动脉粥样硬化的关系

摘　　要：目的观察老年高血压患者的动态脉压与冠心病并发症、颈动脉粥样硬化的关系。方法选取我院2018年10月至2019年10月收治的62老年高血压患者(对照组)与54例老年高血压合并冠心病并发症患者(观察组)作为研究对象,比较两组患者的动态脉压、血脂、凝血因子以及颈动脉粥样硬化检测结果。结果观察组患者24 h平均脉压(24h APP)、白昼平均脉压(dPP)、夜晚平均脉压(nPP)、总胆固醇(TC)、三酰甘油(TG)、高密度脂蛋白胆固醇(HDL-C)均优于对照组,组间差异有统计学意义(P<0.05)。观察组患者平均颈总动脉内径以及颈总动脉内膜中层厚度均明显长于对照组,组间差异有统计学意义(P<0.05)。结论老年高血压合并冠心病患者脉压以及颈动脉粥样硬化斑块严重程度高于老年高血压患者。Objective To observe the relationship between dynamic pulse pressure,coronary heart disease complications and carotid atherosclerosis in elderly patients with hypertension.Methods A total of 62 elderly patients with hypertension(control group)and 54 elderly patients with hypertension and coronary heart disease complications(observation group)were enrolled in our hospital from October 2018 to October 2019.The results of dynamic pulse pressure,blood lipid,coagulation factor and carotid atherosclerosis were compared between the two groups.Results The observation group patients’24-hour mean pulse pressure(24 hours APP),day-time mean pulse pressure(dPP),night-time mean pulse pressure(nPP),total cholesterol(TC),triglycerides(TG),high-density lipoprotein cholesterol(HDL-C)were better than the control group,and the differences between the groups were statistically significant(P<0.05).The average inner diameter of the common carotid artery and the thickness of the intima-media of the common carotid artery in the observation group were significantly longer than those in the control group,and the difference between the groups was statistically significant(P<0.05).Conclusion The severity of pulse pressure and carotid atherosclerotic plaque in elderly patients with hypertension and coronary heart disease is higher than that in elderly patients with hypertension.
