摘 要:目的以组队检测模式调查当前在应对疫情防控中大量培训的新型冠状病毒核酸检测后备人员的技术能力,考查团队检测效率,为后续有重点地提高核酸检测人员技能水平、提升整体应对疫情的检测能力提供数据依据。方法从全省各地市医疗机构、疾控中心和第三方实验室的PCR岗位证名单中按一定比例随机抽调450人,每15人1组,共组建30支组队,按照团队合作模式对每个人的实验技术、检测知识储备以及团队的多个方面进行竞技与考核。按新型冠状病毒核酸检测全过程,分为试剂配制考点、核酸提取A步骤和B步骤考点以及核酸扩增考点,按照每个考点的评分细则给每个选手的实验操作进行评分;设置个人面试,分为PCR结果解读和综合理论论述,对每位选手的理论知识进行考核。结果2020年培训的新证人员各项成绩均低于2019年以前培训的老证人员,且内部差异明显;移液器吸样不准、出现加样错误和手套污染样本的失误率分别为30.30%、21.10%和17.20%,移液器走位不规范、加样超时和未确认加样的失误率分别为34.4%、22.5%和15.0%。组队最终检测结果的准确性与组队的技术成绩呈正相关。队伍前期组织过1~3 d集训的地市成绩显著高于没有组织过集训的地市。结论对2020年新培训的新证人员要加强理论培训,尤其加强PCR结果解读的技术培训。提前开展核酸检测机动队的演习,磨合团队配合,落实技术和流程,确保在疫情出现最初就具备高效检测的能力。Objective To investigate the technical ability of the backup technicians who engaged in novel coronavirus nucleic acid detection and were trained for responding to the epidemic prevention and control by the team mode,and test the efficiency of the team detection,so as to provide data evidence for improving the skill level of technicians engaged in nucleic acid detection and the overall detection ability responding to the epidemic.Methods A total of 450 technicians were randomly selected from the list with certificates for PCR of medical institutions,centers for Diseases Control and Prevention and the third-party laboratories in all cities of Zhejiang province,and 30 teams were formed for each with 15 technicians.The experimental technology,detection knowledge reserve and team spirit of each technician were assessed according to the team cooperation mode.The whole process of new coronavirus nucleic acid detection was divided into reagent preparation,nucleic acid extraction step A and step B,and nucleic acid amplification test sites,and each technician′s experimental operation at each test site was scored.Meanwhile,each technician′s interpretation ability of PCR results and comprehensive theory knowledge were assessed.Results The scores of every test sites of the technicians newly trained in 2020 were lower than those trained before 2019,and the internal differences among the newly trained technicians were significantly higher than those trained before 2019.The error rates of incorrect pipette aspiration,wrong sampling and glove contamination were 30.30%,21.10%and 17.20%,respectively.The error rates of irregular pipette position,overtime sampling and unconfirmed sampling were 34.4%,22.5%and 15.0%,respectively.The accuracy of the final test results of the team was positively correlated with the technical performance of the team.The performances of teams trained for one to three days in advance were significantly higher than those without any training.Conclusion It is necessary to strengthen the theoretical train
