益桑止遗颗粒联合穴位贴敷对脾肾气虚型小儿遗尿的效果

摘　　要：目的探究益桑止遗颗粒联合穴位贴敷治疗脾肾气虚型小儿遗尿的临床效果。方法研究纳入80例脾肾气虚型遗尿症患儿,采取随机数字表法将患儿均分为对照组与治疗组,对照组患儿(40例)采用缩泉胶囊治疗,治疗组患儿(40例)采用益桑止遗颗粒联合穴位贴敷治疗,比较2组患儿的临床疗效、治疗前后中医证候积分(梦中遗尿、夜寐不安、五心烦热、食欲不佳、面色晦暗)及夜间遗尿次数、血浆抗利尿激素(ADH)水平变化、匹兹堡睡眠指数评分(PSQI)及不良反应;随访1年,比较2组患儿治疗6、12个月后遗尿症的复发率。结果治疗组患儿总有效率(80.00%)高于对照组(57.50%),差异有统计学意义(P<0.05);治疗前,2组患儿中医证候积分、夜间遗尿次数、ADH水平、PSQI评分比较,差异均无统计学意义(P>0.05),治疗后2组患儿中医证候积分、夜间遗尿次数、ADH水平、PSQI评分均改善,且治疗组治疗后以上症状改善均优于对照组,差异有统计学意义(P<0.05);2组患儿治疗过程中均未见不良反应发生;治疗6个月后,对照组出现4例复发(10.00%),治疗组2例复发(5.00%);治疗12个月后,对照组患儿出现11例复发(27.50%),治疗组4例复发(10.00%),差异有统计学意义(P<0.05)。结论益桑止遗颗粒联合穴位贴敷治疗脾肾气虚型小儿遗尿临床疗效显著,患儿遗尿等症状改善,睡眠质量改善,未见不良反应,1年后复发率低,安全性高。Objective To explore the clinical effect of Yisang Zhiyi Granules combined with acupoint application in the treatment of infantile enuresis of spleen kidney Qi deficiency type.Methods Eighty children with enuresis of spleen and kidney Qi deficiency type were included in the study.The children were randomly divided into control group and treatment group.The control group(40 cases)were treated with Suoquan Capsules,and the treatment group(40 cases)were treated with Yisang Zhiyi Granules combined with acupoint application.The clinical efficacy,Traditional Chinese Medicine Syndrome Scores(TCMSS,including nocturnal enuresis,nocturnal restlessness,5 item heart trouble,poor appetite,dark complexion),frequency of nocturnal enuresis,changes of plasma antidiuretic hormone(ADH),Pittsburgh Sleep Index Score(PSQI)and adverse reactions were compared between the 2 groups before and after treatment.The recurrence rate of enuresis was compared between the 2 groups after 6 months and 12 months of treatment during the follow-up period of 1 year.Results The total effective rate of the treatment group(80.00%)was higher than that of the control group(57.50%),the difference was statistically significant(P<0.05).Before treatment,the TCMSS,ADH level and PSQI scores of the 2 groups were compared,the difference was not statistically significant(P>0.05);after treatment,TCMSS,ADH level and PSQI scores of each group were improved.After treatment,TCMSS,ADH level and PSQI scores of the treatment group were better than those of the control group,the difference was statistically significant(P<0.05).There were no adverse reactions in the 2 groups;after 6 months of treatment,4 cases recurred in the control group(10.00%)and 2 cases recurred in the treatment group(5.00%);after 12 months of treatment,11 cases recurred in the control group(27.50%),and 4 cases recurred in the treatment group(10.00%),the difference was statistically significant(P<0.05).Conclusion Yisang Zhiyi Granules combined with acupoint application has significant clinical effect in
