单独绑扎钢绳3个8字内固定治疗髌骨骨折的临床应用研究

摘　　要：目的:探讨单独绑扎钢绳3个8字内固定治疗髌骨骨折的临床应用效果。方法:选取2018年5月至2020年5月在惠州仲恺高新区人民医院骨科收治的各类型髌骨骨折手术患者46例,随机分为对照组及观察组。观察组患者手术方式分为单独绑扎钢绳3个8字内固定,对照组患者手术方式为克氏针张力带内固定。术后对比两组患者的临床总有效率、手术指标、术后并发症发生率、患者满意度及AKS评分。结果:术后观察组临床总有效率为100%,较对照组的91.3%显著提高(P<0.05)。观察组手术时间、术中出血量、首次锻炼时间、骨折愈合时间均优于对照组(P<0.05)。观察组患者术后并发症发生率为4.3%,显著低于对照组的13.0%(P<0.05)。观察组患者术后满意度为95.7%,对照组患者术后满意度为87.0%,两组差异显著(P<0.05)。观察组患者的AKS评分优于对照组患者(P<0.05)。结论:单独绑扎钢绳3个8字内固定治疗髌骨骨折的临床疗效良好,可促进患者术后康复,提高治疗满意率,且并发症低,值得临床推广。Objective:To explore the clinical application effect of internal fixation with 38-shaping steel ropes alone in the treatment of patella fractures.Methods:A total of 46 patients undergoing various types of patella fracture surgery who were admitted to the Department of Orthopedics,People’s Hospital of High-tech Industrial Development Zone,Huizhou from May 2018 to May 2020,were randomly divided into the control group and the observation group.The observation group were treated with internal fixation with 38-shaping steel ropes alone,while the control group were treated with internal fixation with Kirschner wire tension bands,with the total clinical effective rates,the surgical indicators,the postoperative complication rates,the patients’satisfaction rates and the AKS scores compared between the two groups after operation.Results:The total clinical effective rate of the observation group after operation was 100%,higher than that of the control group(91.3%)(P<0.05).The operation time,the first time for exercise,and the fracture healing time in the observation group were shorter than those of the control group and the intraoperative blood loss in the observation group was less than that in the control group(P<0.05).The total incidence of postoperative complications in the observation group was 4.3%,lower than that in the control group(13.0%)(P<0.05).The postoperative satisfaction rate of the observation group was 95.7%,higher than that of the control group(87.0%)(P<0.05).The AKS score of the observation group was higher than that of the control group(P<0.05).Conclusion:Internal fixation with 38-shaping steel ropes is very effective in the treatment of patella fractures,which can promote postoperative recovery of patients and the satisfaction rates for treatment and lower complications,worthy of clinical promotion.
