头皮良性肿瘤门诊手术治疗体会

摘　　要：目的探讨不同头皮良性肿瘤的诊断要点及门诊手术方式。方法通过对2018年5月至2019年5月于延安市人民医院神经外科门诊手术室局麻下行包块切除术且经病理学检查确诊的头皮良性肿瘤患者63例进行手术方式总结;结合病理学检查结果,总结不同头皮良性肿瘤的诊断依据。结果门诊诊断与病理学诊断符合率95.2%,2例术前诊断皮脂腺囊肿经病理学检查证实为表皮样囊肿,1例表皮样囊肿误诊为脂肪瘤;63例患者中诊断为脂肪瘤19例、表皮样囊肿15例、皮样囊肿3例、皮脂腺囊肿21例、汗孔瘤2例、钙化上皮瘤2例、海绵状血管瘤1例;均经手术治疗后,预后良好,无一例头皮感染,动态随诊,尚未见复发。结论头皮包块经门诊查体、影像学检查作出初步诊断,不同类型采取不同手术方式,可显著提高手术效果,缩短手术时间,减少并发症。Objective To investigate the diagnostic points and outpatient operation of benign scalp tumors.Methods 63 cases of benign scalp tumor with subanaesthesia resection in outpatient operating room during May 2018 to May 2019 were summarized,combined with pathological Results,summarize the diagnostic basis of benign tumors of different scalp.Results 95.2%of outpatients and pathologic diagnosis,2 cases of preoperative diagnosis of sebaceous glandular cysts were pathologically confirmed as epidermoid cysts.One case of epidermoid cyst was misdiagnosed as lipoma;63 patients with lipoma,19 cases,15 cases of epidermoid cyst,3 cases of dermoid cyst,21 cases of sebaceous gland cyst,2 cases of perspiration pore tumor,2 cases of calcified epithelioma,1 case of cavernous hemangioma.After surgical treatment,the prognosis is good.No scalp infection,dynamic follow-up,no recurrence.Conclusion The initial diagnosis of scalp masses through outpatient physical examination,imaging assistance,and different types of surgical Methods can significantly improve the surgical effect,shorten the operation time,and reduce complications.
