摘 要:目的了解辽宁省成年居民体质指数(BMI)、腰围(WC)和腰围身高比(WHtR)与高血压的关系,为制定有针对性的防控和干预措施提供建议。方法于2016年7—9月采用多阶段分层随机整群抽样方法抽取辽宁省34 889名≥18岁常住居民为研究对象,进行问卷调查和体格检查。应用SPSS 17.0统计软件进行χ^(2)检验和多因素logistic回归分析。结果辽宁省成年居民高血压患病率为26.7%,肥胖率为8.6%,WC和WHtR中心性肥胖率分别为46.4%和40.6%。多因素logistic回归分析结果显示,控制混杂因素后,BMI(超重OR=1.752,肥胖OR=3.573)、WC中心性肥胖(OR=1.639)和WHtR中心性肥胖(OR=1.726)与高血压患病高风险相关(P<0.01)。以BMI正常联合WC正常为对照,不同BMI分级和不同WC水平联合均增加了高血压患病风险(P<0.01);以BMI正常联合WHtR正常为对照,不同BMI分级和不同WHtR水平联合均增加了高血压患病风险(P<0.01)。结论 BMI、WC和WHtR均与高血压的患病关系密切,BMI分别与WHtR和WC的联合应用对高血压患病风险识别具有较高的价值。采取综合干预措施将体重相关指标维持在正常范围对高血压防控具有重要意义。Objective To investigate the relationship between hypertension and body mass index(BMI),waist circumstance(WC)or waist-to-height ratio(WHtR) among adult residents in Liaoning Province,and to provide the suggestion for targeted prevention and control measures. Methods From July to September 2016,the multistage stratified and cluster random sampling method was used to select 34 889 permanent residents (≥18 years old) in Liaoning Province as the subjects. The investigation was conducted by the questionnaire and physical examination. χ^(2) test and multivariate logistic regression were used to analyze the data. The used software was SPSS 17.0. Results The morbidities of hypertension and obesity in adult residents of Liaoning Province were 26.7%and 8.6%,respectively. The central obesity morbidities according to WC and WHtR were 46.4% and 40.6%,respectively. The multivariate logistic regression analysis showed that after controlling confounding factors,BMI(overweight OR=1.752,obesity OR=3.573),WC central obesity(OR=1.639)and WHtR central obesity(OR=1.726) were related to high risk of hypertension(P<0.01).The normal BMI plus normal WC served as the control group,the different BMI levels plus different WC levels can increase the risk of hypertension(P<0.01);if the normal BMI plus normal WHtR served as the control group,also the different BMI levels plus different WHtR levels can increase the risk of hypertension(P <0.01). Conclusion BMI,WC and WHtR are all related to the prevalence of hypertension. The combination of BMI with WC or WHtR could have the higher value for identifying the hypertension risk. Comprehensive intervention measures should be taken for maintaining the normal bodyweight index range,which have the important significance for hypertension prevention and control.
