探讨左胸小切口手术对食管癌患者肺功能及血清炎性因子水平的影响

摘　　要：目的探究食管癌患者实施左胸小切口手术对其肺功能及炎性因子水平的影响分析。方法将该院自2018年1月—2020年2月期间收治的52例食管癌患者采取抽签法随机分为观察组与参照组,每组26例。参照组患者实施传统开胸手术,观察组患者实施左胸小切口手术,对比两组患者肺功能指标及血清炎性因子水平变化。结果观察组患者IL-6为(2.16±0.34)pg/mL、IL-8为(2.25±0.26)pg/mL、TNF-α为(1.24±0.15)pg/mL低于参照组的(2.72±0.41)、(2.69±0.32)、(1.69±0.20)pg/mL,差异有统计学意义(t=5.361、5.441、9.178,P<0.05);观察组的FEV1为(73.48±11.25)%、FVC为(76.58±15.26)%、MVV为(86.73±16.59)%,高于参照组的(65.21±11.57)、(61.43±14.84)、(74.35±11.82)%,差异有统计学意义(t=2.613、3.629、3.099,P<0.05);观察组并发症发生率为3.85%,低于参照组的26.92%,差异有统计学意义(χ2=5.318,P=0.021)。结论左胸小切口手术治疗食管癌能有效改善患者肺功能,降低炎性反应,提高治疗效果。Objective To explore the impact of left chest small incision surgery on lung function and inflammatory factor levels in patients with esophageal cancer.Methods The 52 patients with esophageal cancer admitted to the hospital from January 2018 to February 2020 were randomly divided into a observation group and a reference group,with 26 cases in each group.Patients in the reference group underwent traditional thoracotomy,and patients in the observation group underwent left thoracic small incision surgery.The changes in lung function indexes and serum inflammatory factor levels were compared between the two groups.Results IL-6 was(2.16±0.34)pg/mL,IL-8 was(2.25±0.26)pg/mL,and TNF-αwas(1.24±0.15)pg/mL of the observation group,which was lower than that of the reference group(2.72±0.41)pg/mL,(2.69±0.32)pg/mL,(1.69±0.20)pg/mL,the difference was statistically significant(t=5.361,5.441,9.178,P<0.05);the FEV1 was(73.48±11.25)%,FVC was(76.58±15.26)%and MVV was(86.73±16.59)%,of the observation group,which was higher than the reference group(65.21±11.57)%,(61.43±14.84)%,(74.35±11.82)%,the difference was statistically significant(t=2.613,3.629,3.099,P<0.05);the complication rate in the observation group was 3.85%,which was lower than 26.92%in the reference group,and the difference was statistically significant(χ2=5.318,P=0.021).Conclusion Left chest small incision surgery for esophageal cancer can effectively improve patients'lung function,reduce inflammatory response,and improve treatment effect.
