摘  要:骨性关节炎(Osteoarthritis,OA)是好发于多个年龄段的常见骨关节病,伴有软骨损伤及局部炎症反应等复杂的病理改变。间充质干细胞(Mesenchymal stem cells,MSCs)具有较强的抗炎、组织修复及免疫调节的能力,而间充质干细胞经旁分泌途径分泌的外泌体(Exosome)在组织修复与软骨再生方面具有重要作用。研究发现,MSCs源性外泌体能够通过转运并释放蛋白质、非编码RNA等生物活性物质,介导并传递抗炎及免疫活性因子,缓解OA病理进程,促进软骨修复与再生,但其具体作用机制仍不明确。本文对MSCs源性外泌体在OA治疗中的相关机制研究结果进行综述,以期为OA的临床治疗提供新的方向。Osteoarthritis(OA)is a common osteoarthritis that occurs frequently in multiple age groups,accompanied by complex pathological changes such as cartilage damage and local inflammatory reaction.Mesenchymal stem cells(MSCs)have been proved to have strong anti-inflammatory,tissue repair and immune regulation abilities,and exosome secreted by mesenchymal stem cells through paracrine pathway plays an important role in tissue repair and cartilage regeneration.Studies have found that MSCs-derived exosomes can mediate and deliver anti-inflammatory and immune-active factors,alleviate the pathological process of OA,and promote cartilage repair and regeneration through the transport and release of proteins,non-coding RNA and other bioactive substances,but the specific mechanism is still unclear.In this paper,the recent literatures on the mechanism of MSCs-derived exosomes in the pathogenesis and treatment of OA were reviewed,in order to point out a new direction for the clinical treatment of OA.
