透明质酸钠注射液联合蠲痹汤加减治疗寒湿痹阻型类风湿关节炎活动期患者的临床疗效

摘　　要：目的探讨透明质酸钠注射液联合蠲痹汤加减治疗寒湿痹阻证类风湿关节炎类风湿关节炎(RA)活动期患者的临床疗效。方法选取2019年7月至2020年7月灯塔市中医医院风湿免疫科病房收治的RA活动期患者90例作为研究对象,按随机数字表法分为对照组与观察组,各45例。对照组予以透明质酸钠关节腔注射治疗,观察组在透明质酸钠治疗基础上予蠲痹汤加减,比较两组患者临床疗效、症状体征、实验室指标[红细胞沉降率(ESR)、C反应蛋白(CRP)、类风湿因子(RF)]、生命质量评分[日常生活能力健康评估(HAQ)评分]、中医证候积分。结果观察组治疗有效率为93.3%,高于对照组的66.7%(P<0.05)。治疗后,观察组晨僵时间、关节压痛个数、关节肿胀个数、视觉模拟评分法(VAS)评分、CRP、ESR、RF、HAQ评分及中医证候积分低于对照组,双手握力评分高于对照组,差异有统计学意义(P<0.05)。结论透明质酸钠注射液联合蠲痹汤加减治疗寒湿痹阻型RA活动期疗效确切,患者症状、体征及各指标明显改善,生命质量明显提高,优于单纯透明质酸钠治疗。Objective To explore the clinical effects of sodium hyaluronate injection combined with modified Juanbi Decoction in the treatment of rheumatoid arthritis with cold dampness obstruction syndrome in active stage.Methods From July 2019 to July 2020,a total of 90 patients in active rheumatoid arthritis period admitted to Rheumatology Department Dengta Hospital of Traditional Chinese Medicine were selected as the research objects and randomly divided into a control group and an observation group with 45 cases in each group,according to random number table.The control group was treated with sodium hyaluronate intra-articular injection,and the observation group was treated with modified Juanbi Decoction plus sodium hyaluronate intra-articular injection.The clinical efficacy,symptoms and signs,laboratory indicators[erythrocyte sedimentation rate(ESR),C-reactive protein(CRP),rheumatoid factor(RF)],quality of life scores[Abilities of Daily Living Health Assessment(HAQ)score]between the 2 groups,TCM syndrome points were compared.Results The total effective rate of the control group was 66.7%,which was lower than that of the treatment group(93.3%)(P<0.05).After treatment,the observation group’s morning stiffness,number of joint tenderness,number of joint swelling,visual analogue scale(VAS)score,CRP,ESR,RF,HAQ score and TCM syndrome score were lower than those of the control group,and the hand grip strength score was higher in the control group,the difference was statistically significant(P<0.05).Conclusion Sodium hyaluronate injection combined with modified Juanbi Decoction in the treatment of rheumatoid arthritis with cold dampness obstruction syndrome in active stage has definite curative effects,the symptoms,signs and various indexes of patients are significantly improved,and the quality of life is significantly improved,which is better than that of sodium hyaluronate.
