不同剂量阿托伐他汀联合曲美他嗪对慢性心衰患者心功能及BNP、TNF-α、IL-6的影响

摘　　要：目的探讨分析不同剂量阿托伐他汀联合曲美他嗪对慢性心衰患者心功能及BNP、TNF-α、IL-6的影响。方法在2017年8月~2020年8月间收治的慢性心力衰竭患者中抽取105例作为研究对象,按照随机数字表法分为A、B、C三组,每组35例,三组患者均应用阿托伐他汀联合曲美他嗪治疗,曲美他嗪剂量相同,阿托伐他汀应用剂量分别为10 mg/d、20 mg/d、40 mg/d,比较三组患者治疗前后心脏功能、BNP、TNF-α、IL-6指标水平及药物不良反应发生情况。结果治疗后,B、C组LVEF、6MWD水平均高于A组(P<0.05),且C组高于B组(P<0.05),B、C组LVESd、LVEDd、BNP、TNF-α及IL-6指标水平均低于A组(P<0.05),且C组低于B组(P<0.05),药物不良反应发生率组间比较差异无统计学意义(P>0.05)。结论不同剂量阿托伐他汀联合曲美他嗪应用均可有效改善慢性心衰患者心功能、降低BNP、TNF-α、IL-6水平,但效果并不相同,高剂量(40 mg/d)阿托伐他汀联合曲美他嗪应用效果最佳,且不会增加药物不良反应。Objective To analyze the effects of different doses of atorvastatin plus trimetazidine on cardiac function and BNP,TNF-α,and IL-6 in CHF patients.Methods A total of 105 patients with chronic heart failure treated in our hospital between August 2017 and August 2020 were selected as the study subjects and divided into three groups A,B,and C according to the random number table method,35 patients in each group were treated with atorvastatin combined with trimetazidine at the same dosage,and the patients in the three groups were treated with atorvastatin at the dosage of 10 mg/d,20 mg/d,40 mg/d respectively.Heart function,BNP,TNF-αand IL-6 index levels and occurrence of adverse drug reactions before and after treatment were compared.Results After treatment,LVEF and 6MWD levels in groups B and C were higher than those in group A(P<0.05),and group C was higher than those in group B(P<0.05).LVESd,LVEDd,BNP,TNF-αand IL-6 index levels in groups B and C were lower than those in group A(P<0.05),and group C was lower than those in group B(P<0.05).The incidence of adverse drug reactions between groups was not statistically significant(P>0.05).Conclusion The use of different doses of atorvastatin plus trimetazidine can effectively improve cardiac function and reduce the levels of BNP,TNF-α,and IL-6 in patients with chronic HF,but the effect is not the same,and the high dose(40 mg/d)of atorvastatin plus trimetazidine is most effective and does not increase the adverse drug reaction.
