紫外线光疗联合复方甘草酸铵加消银颗粒治疗银屑病的临床观察

摘　　要：目的探讨紫外线光疗联合复方甘草酸铵加消银颗粒治疗银屑病的临床效果。方法选取2018年5月~2019年9月收治的银屑病患者146例,按入院先后顺序分为参照组和研究组各73例,参照组采用紫外线光疗^(+)维A酸软膏外用治疗,研究组采用紫外线光疗联合复方甘草酸铵+消银颗粒治疗,对比两组治疗前后PASI(银屑病面积和严重程度指数)评分及治疗有效率,同时对比两组治疗前后T淋巴细胞亚群水平及嗜酸性粒细胞P物质、NK1R(神经激肽/速激肽受体1)水平。结果两组治疗前PASI评分差异无统计学意义(P>0.05),治疗后4周、治疗后8周研究组PASI评分均低于参照组,差异明显(P<0.05);研究组有效率94.52%,高于参照组的82.19%,差异明显(P<0.05);两组治疗前CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)T淋巴细胞亚群水平及嗜酸性粒细胞P物质、NK1R水平无较大差异(P>0.05);治疗后CD4^(+)、CD8^(+)、CD4^(+)/CD8^(+)T淋巴细胞亚群水平高于参照组,且嗜酸性粒细胞P物质、NK1R水平低于参照组,差异明显(P<0.05)。结论紫外线光疗联合复方甘草酸铵加消银颗粒治疗银屑病的效果显著。Objective To explore the clinical effect of ultraviolet phototherapy combined with compound ammonium glycyrrhizinate and Xiaoyin granule on psoriasis.Methods 146 patients with psoriasis admitted to our hospital from May 2018 to September 2019 were divided into the reference group and the study group according to the order of admission.The reference group was treated with ultraviolet light therapy and retinoic acid ointment,and the study group was treated with ultraviolet light therapy combined with compound ammonium glycyrrhizinate and Xiaoyin granules.The PASI(psoriasis area and severity index)scores before and after treatment were compared between the two groups at the same time,T lymphocyte subsets,substance P and NK1R of eosinophils were compared before and after treatment.Results There was no significant difference in PASI score between the two groups before treatment(P>0.05).PASI score of the study group was significantly lower than that of the reference group(P<0.05)4 weeks and 8 weeks after treatment;the effective rate of the study group was 94.52%,higher than 82.19%of the reference group,with significant difference(P<0.05);Before treatment,there were no significant differences in the levels of CD4^(+),CD8^(+),CD4^(+)/CD8^(+)T lymphocyte subsets,eosinophil substance P and NK1R between the two groups(P>0.05).After treatment,the levels of CD4^(+),CD8^(+),CD4^(+)/CD8^(+)T lymphocyte subsets were higher than those in the reference group,and the levels of substance P and NK1R in eosinophils were lower than those in the reference group,the differences were significant(P<0.05).Conclusion Ultraviolet phototherapy combined with compound ammonium glycyrrhizinate and Xiaoyin granule has a significant effect on psoriasis and has a high promotion value.
