硕通镜联合输尿管软镜与经皮肾镜治疗肾下盏结石的疗效观察

摘　　要：目的探讨硕通镜联合输尿管软镜碎石术与经皮肾镜碎石术治疗肾下盏结石的疗效。方法2017年1月~2019年1月我科收治肾下盏结石患者107例,随机分为两组。采取硕通镜联合输尿管软镜碎石者53例,为观察组;采取经皮肾镜碎石者54例,为对照组。比较两组的手术时间、术中出血量、住院费用、住院时间、结石清除率、并发症发生率以及视觉模拟疼痛评分(visual analogue scale,VAS),术后随访周期为1个月。结果观察组的术中出血量、住院费用、术后住院时间均显著低于对照组(P<0.05);观察组的手术时间长于对照组(P<0.05),远期结石清除率(术后1月)无统计学差异(P>0.05);观察组并发症以及术后第二天的VAS评分显著低于对照组(P<0.05)。结论硕通镜联合输尿管软镜治疗肾下盏结石临床疗效安全可靠,适合广泛推广,但特殊患者应根据具体病情选择适当手术方式。Objective To explore the curative effect of Shuo Tong ureteroscopy combined with flexible ureteroscopic lithotripsy and percutaneous nephrolithotomy in the treatment of subrenal calyx calculus.Methods From January 2017 to January 2019,107 patients with subrenal calyx calculus were randomly divided into two groups.There were 53 cases underwent Shuo Tong scopy combined with flexible ureteroscopic lithotripsy of the observation group,and 54 cases underwent percutaneous nephrolithotripsy of the control group.The follow-up period was 1 month,and bleeding volume,the cost of hospitalization,the time of hospitalization,the rate of stone removal,complications and visual analog scale(VAS)were compared between the two groups.Results The bleeding volume,hospitalization cost and hospitalization time of the observation group were significantly lower than that of the control group(P<0.05);and the operation time of the observation group was longer than that of the control group(P<0.05).There was no statistical difference in the long-term stone removal rate(P>0.05);and the VAS score of the observation group was significantly lower than that of the control group(P<0.05).Conclusion The combination of Shuo Tong scopy and flexible ureteroscopic lithotripsy is safe and reliable in the treatment of subrenal calyx calculus,and it is suitable for wide spread.But special patients should choose appropriate operation methods according to the specific conditions.
