摘　　要：目的探讨医院-社区-家庭三元联动延续性护理模式在老年宫颈癌术后发生尿潴留的出院患者中的应用效果。方法选取2018年1月-2018年12月宫颈癌术后发生尿潴留出院的老年患者,共90例,随机分为对照组和观察组,每组45例。对照组给予常规出院宣教及随访,观察组采用三元联动延续性护理干预措施。在出院2周、1个月、3个月与6个月后,评价2组膀胱功能恢复情况及患者自我管理测评得分。结果2组患者出院3个月、6个月后自我管理测评效果显著,观察组残余尿量、尿潴留发生率、尿路感染发生率以及重新置管率均低于对照组,差异有统计学意义(P<0.05)。结论医院-社区-家庭三元联动延续性护理有利于老年宫颈癌根治术后患者顺利完成医院到家庭的过渡,提升患者生活自理能力,促进膀胱功能的恢复、减少尿潴留及尿路感染的发生。Objective To explore the effects of hospital-community-family ternary continuous nursing mode in elderly discharged patients with urinary retention after cervical cancer surgery.Methods A total of 90 elderly discharged patients with urinary retention after cervical cancer surgery from January 2018 to December 2018 were selected,and they were divided randomized into observation group and control group,with 45 cases in each group.The control group was given routine discharge education and follow-up,and the observation group was given hospital-community-family ternary continuous nursing intervention.At 2 weeks,1 month,3 months and 6 months after discharge,the recovery condition of bladder function of the patients and patient self-management evaluation scores were evaluated.Results At 3 and 6 months after discharge,the patient self-management evaluation results of the two groups improved significantly.The residual urine volume,the incidence of urinary retention and urinary tract infection,and reinsertion rate of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05).Conclusion Hospital-community-family ternary continuous nursing is conducive to the smooth transition of elderly patients after radical operation of cervical cancer from hospital to family,enhance their self-care ability,promote the recovery of bladder function,and reduce urinary retention and urinary tract infections.
