单孔后腹腔镜肾切除术对无功能积水肾患者术后疼痛的影响

摘　　要：目的:分析单孔后腹腔镜肾切除术对无功能积水肾患者术后疼痛的影响。方法:回顾性分析2018年1月—2020年1月南阳市第二人民医院收治的80例无功能积水肾患者的临床资料,根据治疗方法不同分为对照组(n=40)和观察组(n=40),对照组采用标准后腹腔镜肾切除术治疗,观察组采用单孔后腹腔镜肾切除术治疗,比较两组围术期指标、疼痛程度(VAS)评分、切口美观满意度及并发症发生率。结果:观察组胃肠道恢复时间和留置引流管时间均短于对照组(P<0.05),两组手术时间、术中出血量及住院时间比较差异无统计学意义(P>0.05);观察组术后6 h、24 h及48 h的VAS评分均低于对照组(P<0.05);观察组切口美观满意度高于对照组(97.50%VS 82.50%)(P<0.05);两组并发症发生率比较差异无统计学意义(P>0.05)。结论:单孔后腹腔镜肾切除术切口美观,可缓解无功能积水肾患者术后疼痛程度,且安全性较高。Objective:To analyze the effect of single hole retroperitoneal laparoscopic nephrectomy on postoperative pain in patients with nonfunctional hydronephrosis.Methods:The clinical data of 80 patients with nonfunctional hydronephrosis admitted to the hospital from January 2018 to January 2020 were analyzed retrospectively.According to different treatment methods,they were divided into control group(n=40)and observation group(n=40).The control group was treated with standard retroperitoneal laparoscopic nephrectomy,and the observation group was treated with single retroperitoneal laparoscopic nephrectomy.The perioperative indexes,VAS score,satisfaction of incision beauty and incidence of complications were compared between the two groups.Results:The recovery time of gastrointestinal tract and the time of indwelling drainage tube in the observation group were shorter than those in the control group(P<0.05).And there was no statistically significant difference between the two groups in operation time,intraoperative hemorrhage and hospitalization time(P>0.05).The VAS score of the observation group at 6 h,24 h and 48 h after operation were lower than those in the control group(P<0.05).The satisfaction of incision aesthetics in the observation group was higher than that in the control group(97.50%vs 82.50%),(P<0.05).There was no statistically significant difference in the incidence of complications between the two groups(P>0.05).Conclusion:Single port retroperitoneal laparoscopic nephrectomy has a beautiful incision,it can relieve postoperative pain in patients with non-functioning hydronephrosis and is safer.
