宫腔镜在围绝经期异常子宫出血诊断中的应用

摘　　要：目的:探讨宫腔镜在围绝经期异常子宫出血诊断中的应用价值。方法:选择2018年5月—2019年8月洛阳仁大医院收治的围绝经期异常子宫出血患者100例,所有患者均采用病理学、宫腔镜检测。以病理学诊断结果为“金标准”,评估宫腔镜诊断围绝经期异常子宫出血病因的符合率。结果:100例围绝经期异常子宫出血患者中,经病理学检测正常子宫内膜4例(4.00%),子宫内膜增生20例(20.00%),子宫内膜癌19例(19.00%),子宫内膜息肉22例(22.00%),子宫内膜炎11例(11.00%),子宫黏膜下肌瘤24例(24.00%);以病理学诊断结果为“金标准”,宫腔镜诊断围绝经期异常子宫出血病因的符合率为94.79%(91/96),且宫腔镜诊断围绝经期异常子宫出血病因符合率与病理学诊断结果具有极好一致性(Kappa=0.934)。结论:宫腔镜诊断围绝经期异常子宫出血病因与病理学诊断结果具有较高的诊断符合率,且检查创伤性较小,可为临床诊断围绝经期异常子宫出血病因及治疗方案制定提供重要依据。Objective:To explore the value of hysteroscopy in the diagnosis of abnormal uterine bleeding during perimenopausal period.Methods:A total of 100 patients with perimenopausal abnormal uterine bleeding treated in the hospital from May 2018 to August 2019 were selected.All patients were examined by pathology and hysteroscopy.The pathological diagnosis results were used as the“gold standard”to evaluate the coincidence rate of hysteroscopy in the diagnosis of abnormal uterine bleeding during perimenopausal period.Results:Among 100 patients with perimenopausal abnormal uterine bleeding,4 cases(4.00%)were normal endometrium,20 cases(20.00%)were endometrial hyperplasia,and 19 cases(19.00%)were endometrial cancer,22 cases(22.00%)were endometrial polyps,11 cases(11.00%)were endometritis,24 cases(24.00%)were uterine submucosal fibroids.The pathological diagnosis results were used as the“gold standard”,the coincidence rate of hysteroscopy in diagnosing the cause of abnormal uterine bleeding in perimenopausal period was 94.79%(91/96),and the coincidence rate of hysteroscopy in diagnosing the cause of abnormal uterine bleeding in perimenopausal period was very consistent with the pathological diagnosis results(Kappa=0.934).Conclusion:Hysteroscopy for the diagnosis of abnormal uterine bleeding in perimenopausal period has a high diagnostic coincidence rate with pathological diagnosis results,and the inspection is less traumatic,which can provide an important basis for clinical diagnosis of the cause and formulation of treatment plan of abnormal uterine bleeding in perimenopausal period.
