孕期甲亢患者血清25羟-维生素D水平与甲状旁腺激素变化的相关性

摘　　要：目的:探讨孕期甲亢患者血清25羟-维生素D(25-OH-D)水平与甲状旁腺激素(PTH)变化的相关性。方法:回顾性收集2018年2月—2020年2月辉县市人民医院收治的200例孕期甲亢患者临床资料纳入观察组(n=200),并选取同期于我院进行健康产检的200例健康孕妇体检资料纳入对照组(n=200),所有受检者均接受25-OH-D及PTH水平检测,比较两组孕期甲亢患者血清25-OH-D水平与PTH变化的相关性。结果:观察组平均血清25-OH-D水平为(53.28±11.57)nmol/L,对照组为(76.28±19.81)nmol/L,组间相比,差异有统计学意义(t=14.1785,P<0.001);观察组孕早、中、晚PTH水平及差值均较对照组高,差异有统计学意义(P<0.05);入选200例甲亢患者检查结果显示,54例血清25-OH-D水平充足,占比27.00%,平均25-OH-D水平为(78.29±19.36)nmol/L;146例血清25-OH-D水平低下,占比73.00%,平均25-OH-D水平为(53.68±11.39)nmol/L,组间相比,差异有统计学意义(t=11.056,P<0.001);维生素D充足组平均血清PTH水平为(75.36±11.41)pg/ml,维生素D低下组为(90.14±20.37)pg/ml,组间相比,差异有统计学意义(t=5.042,P<0.001);采用双变量Pearson直线相关性分析发现,孕期甲亢患者血清25-OH-D水平与PTH水平呈负相关(r=-0.612,P<0.001)。结论:孕期甲亢患者血清25-OH-D水平与PTH呈负相关,临床可通过对25-OH-D、PTH水平的检测,有效评估患者病情,为临床实施对症且有效的方案提供依据。Objective:To investigate the correlation between serum 25 hydroxy vitamin D(25-OH-D)level and parathyroid hormone(PTH)in pregnant women with hyperthyroidism.Methods:The clinical data of 200 pregnant women with hyperthyroidism in the hospital from February 2018 to February 2020 were retrospectively collected and included in the observation group(n=200),and the physical examination data of 200 healthy pregnant women in the hospital during the same period were selected as the control group(n=200).All the subjects were tested for 25-OH-D and PTH levels.The correlation between serum 25-OH-D levels and PTH changes in the two groups was compared.Results:The average serum 25-OH-D level of the observation group was(53.28±11.57)nmol/L,and that of the control group was(76.28±19.81)nmol/L;the difference between the two groups was statistically significant(t=14.1785,P<0.001).The PTH level and difference in the early,middle and late pregnancy of the observation group were higher than those of the control group,the difference was statistically significant(P<0.05).The examination results of 200 patients with hyperthyroidism showed that 54 patients had sufficient serum 25-OH-D levels,accounting for 27.00%,with an average 25-OH-D level of(78.29±19.36)nmol/L.146 cases of serum 25-OH-D level Low,accounting for 73.00%,the average 25-OH-D level is(53.68±11.39)nmol/L,the difference between the two groups was statistically significant(t=11.056,P<0.001),The average serum PTH level was(75.36±11.41)pg/ml in the vitamin D sufficient group,and(90.14±20.37)in the low vitamin D group.There was statistically significant difference between the two groups(t=5.042,P<0.001).The bivariate Pearson linear correlation analysis showed that the serum 25-OH-D level was negatively correlated with PTH level(r=-0.612,P<0.001).Conclusion:The serum 25-OH-D level is negatively correlated with PTH in pregnant women with hyperthyroidism.The detection of 25-OH-D and PTH levels can effectively evaluate the patients’condition and provide the basis for clinic
