摘 要:目的调查医院重症监护病房(ICU)中耐甲氧西林金黄色葡萄球菌(MRSA)感染患者周围环境物体表面污染状况,为感染防控提供依据。方法通过现场采样和分离鉴定技术,对上海某医院2019年1—12月入住ICU的MRSA感染患者周围环境物体表面金黄色葡萄球菌(SA)污染状况进行调查分析。结果该医院ICU在2019年共入住MRSA感染患者13例,采集环境物体表面样品143份,检出SA24株,检出率为16.78%。高频接触物品表面SA检出率为44.83%,高于中频和低频接触物品;重复使用的抹布擦拭后检出率高于使用一次性消毒湿巾。Logistic回归模型分析显示,患者年龄>80岁、高频接触物品是环境表面SA检出的危险因素,使用一次性湿巾对表面进行擦拭可能是其保护因素。结论MRSA感染患者周围环境表面SA污染率较高,高频接触物品表面污染更严重,需进一步规范医疗机构抹布的使用。Objective To investigate the contamination of environmental object surfaces around patients with methicillin-resistant Staphylococcus aureus(MRSA)infection in Intensive Care Unit(ICU)of a hospital,and to provide a basis for infection prevention and control.Methods Through on-site sampling and separation and identification technology,the status of Staphylococcus aureus(SA)contamination on the environmental object surfaces around MRSA-infected patients admitted to the ICU of a Shanghai hospital from January to December of 2019 was investigated and analyzed.Results A total of 13 patients with MRSA infection were admitted to the hospital’s ICU in 2019.143 samples of environmental object surfaces were collected,and 24 strains of SA were detected,with a detection rate of 16.78%.The detection rate of SA on the surfaces of high-frequency contact objects was 44.83%,which was significantly higher than that of medium-frequency and low-frequency contact object surfaces;and the detection rate of re-used duster cloth after wiping was higher than that of disposable disinfectant wipes.Logistic regression model analysis showed that patients’age>80 years and high-frequency contact object surfaces were risk factors for the detection of SA on environmental object surfaces.Wiping the surface with disposable disinfectant wipes might be its protective factor.Conclusion The contamination rate of SA on the environmental object surfaces around patients with MRSA infection is higher.The contamination on the surfaces of high-frequency contact objects is more serious,and the use of wipes in medical institutions needs to be further regulated.
