摘 要:目的:观察高频超声在微针刀治疗腰背部肌筋膜疼痛综合征(MPS)中的应用效果。方法:选取74例腰背部MPS患者,按随机数字表法分为观察组和对照组各37例。对照组采用常规微针刀治疗,观察组在高频超声引导下使用微针刀治疗,2组均隔2天治疗1次,共治疗3次。比较2组临床疗效及治疗前后超声检查结果、视觉模拟评分法(VAS)评分和下腰痛评估系统(JOA)评分。结果:观察组总有效率为94.59%,高于对照组的82.97%(P<0.05)。治疗后,2组肌筋膜激痛点(MTrPs)弹性评分均较治疗前降低(P<0.05),血流速度较治疗前增大(P<0.05);观察组MTrPs弹性评分低于对照组(P<0.05),血流速度大于对照组(P<0.05);2组VAS评分均较治疗前降低(P<0.05),观察组VAS评分低于对照组(P<0.05);2组JOA评分均较治疗前升高(P<0.05),观察组JOA评分高于对照组(P<0.05)。结论:高频超声引导下实施微针刀治疗MPS,能够提高临床疗效,有效改善MTrPs弹性和血流速度,减轻疼痛,提高腰部功能。Objective: To observe the effect of application of high-frequency ultrasound in micro acupotomy treating lumbodorsal myofascial pain syndrome(MPS).Methods:A total of 74 cases of patients with lumbodorsal MPS were selected and divided into the observation group and the control group according to the random number table method,37 cases in each group. The control group was given routine micro acupotomy, and the observation group was additionally given micro acupotomy guided by high-frequency ultrasound.In the two groups,the treatment was carried out once every three days,for a total of three times. The clinical effect was compared between the two groups. Before and after treatment, the ultrasonography result as well as scores of Visual Analogue Scale(VAS) and Japanese Orthopaedic Association(JOA) for pain in low back was compared between the two groups. Results: The total effective rate was 94.59% in the observation group,higher than that of 82.97% in the control group(P<0.05).After treatment,the elastic scores of myofascial trigger points(MTrPs) in the two groups were decreased when compared with those before treatment(P<0.05), and blood flow velocities were increased(P<0.05).After treatment,the elastic score of MTrPs in the observation group was lower than that in the control group(P<0.05),and blood flow velocity was faster(P<0.05).After treatment,scores of VAS in the two groups were decreased when compared with those before treatment(P<0.05),and the score in the observation group was lower than that in the control group(P<0.05);scores of JOA in the two groups were increased when compared with those before treatment(P<0.05),and the score in the observation group was higher than that in the control group(P<0.05). Conclusion:In the treatment of MPS,micro acupotomy guided by high-frequency ultrasound can enhance clinical effect and the lumbar function,effectively improve elasticity of MTrPs and blood flow velocity,and relieve pain.
