摘  要:目的观察喘息婴幼儿血清YKL-40和载脂蛋白C1(apolipoprotein C1,APOC1)水平,探讨其在婴幼儿喘息中的作用。方法喘息急性发作患儿69例,其中首次喘息发作患儿35例为首次喘息组,喘息发作≥2次患儿34例为反复喘息组;同期行疾病筛查后的健康儿童30例为对照组;根据6岁以下儿童哮喘诊断标准模型评分,将69例患儿分为哮喘阳性组(≥4分)24例和哮喘阴性组(<4分)45例。采用ELISA法检测各组血清YKL-40和APOC1水平,并进行比较;采用Spearman秩相关分析喘息患儿血清YKL-40与APOC1水平的相关性。结果反复喘息组和首次喘息组血清YKL-40[30.97(23.08,37.71)、20.90(10.05,31.90)μg/L]、APOC1[142.70(96.77,244.53)、145.99(101.99,262.95)μg/L]水平均高于对照组[12.85(9.02,15.22)μg/L、104.91(85.93,134.59)μg/L](P<0.05),反复喘息组血清YKL-40水平高于首次喘息组(P<0.05),血清APOC1水平与首次喘息组比较差异无统计学意义(P>0.05);哮喘阳性组患儿血清YKL-40水平[31.13(23.77,37.06)μg/L]高于哮喘阴性组[21.42(11.84,36.68)μg/L](P<0.05),血清APOC1水平[148.71(95.05,261.47)μg/L]与哮喘阴性组[142.58(103.28,231.10)μg/L]比较差异无统计学意义(P>0.05)。血清YKL-40水平与APOC1水平呈正相关(rs=0.290,P=0.016)。结论血清YKL-40和APOC1在喘息婴幼儿中水平升高,可能参与婴幼儿喘息的急性气道炎性反应过程,其中血清YKL-40水平升高可能是影响婴幼儿哮喘发生和反复喘息的因素。Objective To observe the changes and significances of serum YKL-40 and apolipoprotein C1(APOC1)and to investigate their roles in infants and toddlers with wheezing.Methods Sixty-nine infants and toddlers with acute attack of wheezing were divided into 35 patients with first attack of wheezing(first attack group)and 34 patients with twice or more times recurrent wheezing(recurrent group),while another 30 healthy infants and toddlers receiving disease screening were as controls(control group).According to the standard model score for the diagnosis of asthma in patients under 6 years,69 patients were divided into 24 patients with standard model score of≥4(asthma positive group)and 45 patients with standard model score of<4(asthma negative group).The levels of serum YKL-40 and APOC1 were detected by ELISA and compared.Spearman rank correlation was used to analyze the correlation between YKL-40 and APOC1.Results The levels of serum YKL-40 and APOC1 were higher in recurrent group(30.97(23.08,37.71),142.70(96.77,244.53)μg/L)and first attack group(20.90(10.05,31.90),145.99(101.99,262.95)μg/L)than those in control group(12.85(9.02,15.22),104.91(85.93,134.59)μg/L)(P<0.05),the serum level of YKL-40 was higher in recurrent group than that in first attack group(P<0.05),and the serum level of APOC1 showed no significant difference between recurrent group and first attack group(P>0.05).The level of serum YKL-40 was higher in asthma positive group(31.13(23.77,37.06)μg/L)than that in asthma negative group(21.42(11.84,36.68)μg/L)(P<0.05).There was no significant difference in serum APOC1 level between asthma positive group(148.71(95.05,261.47)μg/L)and asthma negative group(142.58(103.28,231.10)μg/L)(P>0.05).The serum YKL-40 level was positively correlated with the level of APOC1(rs=0.290,P=0.016).Conclusion The elevated levels of serum YKL-40 and APOC1 may be involved in the process of acute airway inflammation in infants and toddlers with wheezing,and the elevated level of serum YKL-40 may be an influencing factor of ast
