推拿联合补中益气汤加减治疗小儿脑瘫特发性便秘的疗效观察

摘　　要：目的观察推拿联合补中益气汤加减治疗小儿脑瘫特发性便秘的疗效。方法选取2019年1月—2020年8月期间秦皇岛市妇幼保健院收治的脑瘫特发性便秘患儿88例,按照随机数字表法分为对照组44例与治疗组44例。对照组给予常规西医治疗,治疗组在对照组基础上给予推拿联合补中益气汤加减治疗。两组均治疗2周后,对比两组患儿中医证候疗效,综合疗效,治疗安全性及治疗前后便秘改善情况(排便频率、排便困难度及大便性状评分),免疫功能(CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+))变化。结果治疗后治疗组证候疗效总有效率90.70%(39/43),高于对照组75.00%(33/44),两组比较,差异有统计学意义(P<0.05)。治疗后治疗组综合疗效总有效率为93.02%(40/43),优于对照组75.00%(33/44),两组比较,差异有统计学意义(P<0.05)。治疗后两组患儿排便频率、排便困难度及大便性状评分均较治疗前改善(P<0.05),且治疗组优于对照组(P<0.05)。治疗后两组患儿CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均较治疗前升高(P<0.05),且治疗组高于对照组(P<0.05)。治疗期间两组患儿均无不良反应发生。结论推拿联合补中益气汤加减治疗小儿脑瘫特发性便秘疗效显著,可有效缓解便秘症状,提高患儿免疫功能,且安全性具有保障。Objective To explore the application value of massage combined with modified Buzhong Yiqi Decoction in children with idiopathic constipation of cerebral palsy.Method A total of 88 children with idiopathic constipation of cerebral palsy who were admitted to Qinhuangdao Maternity and Child Health Hospital from January 2019 to August2020 were selected and divided into a control group of 44 cases and a treatment group of 44 cases according to a random number table.The control group was treated with conventional Western medicine,and the treatment group was treated with massage combined with modified Buzhong Yiqi Decoction on this basis,both groups were treated for 2 weeks.The curative effect of TCM syndromes,comprehensive curative effect,treatment safety,improvement of constipation before and after treatment,and changes of immune function[T lymphocyte subset levels(CD3^(+),CD4^(+),CD4^(+)/CD8^(+))]were compared between the two groups.Result After treatment,the total effective rate of syndromes in the treatment group was 90.70%(39/43),which was higher than 75.00%(33/44)in the control group.The difference between the two groups was statistically significant(P<0.05).After treatment,the total effective rate of the treatment group was 93.02%(40/43),which was better than the control group by 75.00%(33/44).The difference between the two groups was statistically significant(P<0.05).After treatment,the frequency of defecation,difficulty in defecation and the scores of stool traits in the two groups were improved compared with those before treatment(P<0.05),and the treatment group was better than the control group(P<0.05).After treatment,the levels of CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in the two groups were higher than before treatment(P<0.05),and the treatment group was higher than the control group(P<0.05).During the treatment,there were no adverse reactions in the two groups of children.Conclusion Massage combined with Buzhong Yiqi Decoction is effective in treating idiopathic constipation in children with cerebral palsy.It can
