摘 要:目的探讨学龄儿童身体发育与甲状腺容积的关系,为甲状腺肿大的判定提供科学依据。方法在北京市丰台区随机抽取407名8~10岁儿童,通过问卷调查收集学龄儿童的一般信息,按照统一标准进行学龄儿童的身高、体重测量,收集儿童随机尿样和家中食用盐检测碘含量,B超检测确定儿童甲状腺容积。应用广义线性回归方法评估身体发育指标(身高/体重/体质指数/体表面积)对甲状腺容积、甲状腺肿大的影响。结果2018-2019年北京市丰台区8~10岁学龄儿童的平均身高、体重、体质指数和体表面积分别为(141.4±6.9)cm、(35.4±7.8)kg、(17.6±3.1)kg/m^(2)和(1.29±0.13)m^(2),儿童甲状腺容积平均为(3.94±1.09)mL,甲状腺肿大率为12.8%。身体发育指标与甲状腺容积、甲状腺肿大的回归模型结果显示,随着身体发育指标增加,甲状腺容积增大,被判定为甲状腺肿大的概率增加,其中体表面积的影响最大。在男生中身体发育指标对甲状腺容积和肿大的影响更大。结论学龄儿童甲状腺容积与身体发育密切相关,为科学、准确衡量人群碘营养水平,进行甲状腺肿大判定时应考虑身体发育对其的影响。Objective To explore the association between physical development and thyroid volume in school-aged children,in order to provide a more scientific basis for judging goiter.Methods A total of 407 children aged 8-10 years were selected using random sampling method in Fengtai district,Beijing.General information was collected by questionnaire,height and weight were measured according to unified standards.Iodine concentration determination,random urine specimen and cooking salt samples of participants were collected meanwhile.Thyroid volume was examined by B-ultrasonography.Generalized linear regression method was used to explore the association between physical development indicators[height/weight/body mass index(BMI)/body surface area(BSA)]and thyroid volume,as well as goiter.Results For the children aged 8-10 years in Fengtai district,Beijing,the average height,weight,body mass index,and body surface area were(141.4±6.9)cm,(35.4±7.8)kg,(17.6±3.1)kg/m^(2),and(1.29±0.13)m^(2),respectively.The average thyroid volume was(3.94±1.09)mL with a goiter rate of 12.8%.According to the regression model,thyroid volume and the chance of being judged as goiter increased with physical development increasing among school-aged children,with body surface area having the greatest effect.The effects were greater in boys.Conclusions the thyroid volume is closely related to physical development in school-aged children.In order to scientifically and accurately measure the level of iodine nutrition in the population and assess thyroid volume and goiter,the impact of physical development should be considered.
