高兆旺辨治阴囊潮湿临床经验

摘　　要：阴囊潮湿是男科常见症状,可出现在前列腺炎、精索静脉曲张等多种疾病中,也可作为独立症状存在,多伴有异味或瘙痒等。山东中医药大学附属医院男科高兆旺教授对该疾病辨证论治,认为阴囊潮湿表现为阴囊部出汗,病位虽在阴囊局部,却与心、肝、脾、肺、肾密切相关,与今人饮食作息相关,常见病因为辛辣饮食、久坐、体虚、寒湿之邪等,阴囊潮湿病理因素多为湿、热、瘀,病机主要是湿热循经、寒湿侵体、阳虚不能化湿、阴虚不能制阳、瘀血日久化热等,可按照虚实辨证,实证多见肝经湿热证、寒湿凝滞证、气滞血瘀证,虚证多见肾阳虚衰证、阴虚火旺证、气虚血瘀证。我们应结合症状辨证论治,酌情加减药物。肝经湿热者清利湿热,寒湿凝滞者温阳化湿,气滞血瘀者理气活血化瘀,肾阳虚衰者温阳化湿,阴虚火旺者滋阴降火,气虚血瘀者补气活血,分别运用龙胆泻肝汤加减,苓桂术甘汤合吴茱萸汤加减,少腹逐瘀汤加减,安肾丸加减,知柏地黄丸加减,补中益气汤合少腹逐瘀汤加减。在治疗阴囊潮湿尤其是湿热证、寒湿证和血瘀证时,善用荆芥、防风等风药,以获得更好疗效。Scrotal wetness is a common symptom of andrology. It can occur in many diseases such as prostatitis, varicocele and other diseases. It can also exist as an independent symptom, often accompanied by odor or pruritus. Professor Gao Zhaowang of Andrology of Affiliated Hospital of Shandong University of Traditional Chinese Medicine treats the disease based on dialectical treatment.He believes that the scrotum is wet and sweating. Although the disease is localized in the scrotum, it is closely related to the heart, liver,spleen, lung, and kidney. Its common etiology includes spicy diet, sedentary, weak body, cold and dampness, etc. The pathological factors of scrotal wetness are mostly damp, heat, and blood stasis. The pathogenesis is mainly hot and humid circulation, cold and wet invasion,and Yang( 阳) deficiency fails to remove dampness and Yin( 阴) deficiency fails to control Yang and long blood stasis produces heat.Excess pattern is dampness-heat in the liver meridian, syndrome of internal obstruction of cold-dampness, and blood stasis due to Qi(气)stagnation. Deficiency pattern is kidney Yang deficiency, Yin deficiency with effulgent fire, Qi deficiency with blood stasis. We should diagnose the pattern through comprehensive analysis and add or subtract drugs as appropriate. Those with dampness and hotness in the liver should clear heat and resolve dampness, those with coldness and dampness should warm Yang and resolve dampness, those with Qi stagnation and blood stasis should regulate Qi and activate blood, those with kidney Yang deficiency should warm Yang and resolve dampness, those with Yin deficiency and fire prospering should nourish Yin and expel fire, those with Qi deficiency and blood stasis should nourish Qi and activate blood, Modified Longdan Xiegan decoction( 龙胆泻肝汤), Linggui Zhugan decoction( 苓桂术甘汤),modified Wuzhuyu decoction(吴茱萸汤), Modified Shaofu Zhuyu decoction(少腹逐瘀汤), Modified Anshen pill(安肾丸), modified Zhibai Dihuang decoction( 知 柏 地 黄 丸),
