摘  要:目的:研究自动追踪冻结(SSF)联合智能边缘修复(IBR)技术在宝石能谱CT冠状动脉成像诊断心肌桥(MB)中的应用价值。方法:选取医院收治的468例疑似冠心病患者,依据是否存在心肌桥将468例患者分为MB组(152例)和非MB组(316例),再将MB组分为心率正常组(64例)和心率异常组(88例)。所有患者均行宝石能谱CT冠状动脉检查,根据SSF和IBR技术重建图像并由经验丰富的CT医师采用双盲法进行分析。结果:468例疑似冠心病患者中宝石能谱CT冠状动脉成像共检查出MB患者152例,检出率为32.48%(152/468)。152例MB患者检查出冠状动脉174段,与SSF技术重建相比,IBR技术重建后冠状动脉狭窄度、图像评分显著升高(t=4.672,t=2.221;P<0.05);与单独SSF和IBR技术重建相比,SSF联合IBR技术重建后冠状动脉狭窄度、图像评分显著升高(t=10.111,t=5.609,t=4.706,t=2.393;P<0.05);与心率异常组相比,心率正常组图像评分显著升高(t=3.052,P<0.05)。MB患者和非MB患者在平均年龄、性别、心率方面相比差异无统计学意义。结论:SSF联合IBR技术重建可提高宝石能谱CT冠状动脉成像图像质量,具有一定的临床应用价值。Objective:To study the application value of snap shot freeze(SSF)combined with intelligent boundary registration(IBR)in the diagnosis of myocardial bridging(MB)by coronary angiography with gem energy spectrum computed tomography(CT).Methods:468 patients with suspected coronary heart disease(CHD)admitted to hospital were selected,and they were divided into MB group(152 cases)and non-MB group(316 cases)according to whether there was a myocardial bridge.And MB group was further divided into normal heart rate group(64 cases)and abnormal heart rate group(88 cases).All patients underwent coronary artery examination with gem energy spectrum CT.The images were reconstructed according to SSF and IBR techniques,and double-blind method was adopted to analyze these images by CT physician with rich experience.Results:In 468 patients with suspended CHD who underwent the coronary angiography examination with gem energy spectrum CT,152 patients with MB were confirmed by this method and the detectable rate was 32.48%(152/468).174 segments of coronary artery were found in 152 patients with MB.Compared with the reconstruction of SSF technique,the stenosis degree and image score of coronary artery of the reconstruction with IBR technique were significantly increased(t=4.672,t=2.221,P<0.05).Compared with the reconstructions of single SSF and IBR technique,the stenosis degree and image score of coronary artery of the reconstruction of the combined SSF and IBR technique were significantly increased(t=10.111,t=5.609,t=4.706,t=2.393,P<0.05).The image score of normal heart rate group was significantly higher than that of abnormal heart rate group(t=3.052,P<0.05).The differences of average age,gender and heart rate between MB patients and non-MB patients were no significant.Conclusions:The reconstruction of SSF combined with IBR can improve the image quality of coronary angiography with gem energy spectrum CT,which has a certain of clinical application value.
