血型抗体检测对孕产妇产前备血及新生儿溶血病预防的回顾性分析

摘　　要：目的探讨孕产妇抗体筛查对产前备血及新生儿溶血病预防的意义。方法选取2018年1月至2020年5月首都医科大学附属北京友谊医院行孕妇产前检查的8347例孕产妇,均进行血型鉴定、抗体筛查、抗体鉴定,抗体效价测定、抗人球蛋白试验等血型血清学检测。根据抗体鉴定结果并选择其对应抗原阴性的血交叉配血,对有抗体的新生儿留脐血并进行溶血3项试验,预测新生儿溶血病。结果抗体筛查阳性32例(0.38%);鉴定血型抗体为抗-E、抗-c和抗-M等,及罕见的抗-Fya抗体1例,分别选择相应抗原阴性的血给予备血;新生儿溶血3项试验阳性15例,根据新生儿溶血病严重程度给予换血或输血治疗。结论孕期抗体筛查并鉴定抗体,对产前备血及预测新生儿溶血病尤为重要。Objective To investigate the maternal blood group antibody screening on prenatal blood preparation and prevention hemolytic disease of the newborn(HDN).Methods A total of 8347 cases of pregnant women who underwent prenatal examination in Beijing Friendship Hospital from January 2018 to May 2020 were selected for blood group serological detection,including blood typing,antibody screening,antibody identification and antibody titer determination.According to the results of antibody identification,the corresponding antigen negative blood was selected for cross matching.The umbilical cord blood was collected from the newborns for three hemolytic tests in order to predicting neonatal hemolytic disease.Results Thirty-two cases(0.38%)positive antibody were found through antibody screening.A variety of blood type antibody were identified,such as anti-c,anti-E and anti-M,especially rare antibody Fya.Corresponding antigen negative blood were respectively chosen for the maternal.Fifteen cases were positive in the neonatal hemolysis test and blood exchange or blood transfusion was given according to the severity of HDN.Conclusions Antibody screening and identification of antibodies are important for pregnant women in late pregnancy,particularly for prenatal blood preparation and predicting HDN.
