脑电双频指数监测下全麻妇科患者丙泊酚用量与血红蛋白之间的相关性

摘　　要：目的探讨在脑电双频指数(BIS)监测下全麻妇科患者丙泊酚用量与血红蛋白之间的相关性。方法选取择期妇科腹腔镜手术患者48例,年龄35~55岁,ASAⅠ或Ⅱ级,体质量指数18~24 kg·m^(-2),采用脑电双频指数监测下丙泊酚全身麻醉。根据术前血红蛋白含量将入选患者分成正常组、轻度贫血组和中度贫血组,比较3组患者的一般资料、手术麻醉时间和丙泊酚使用量。结果 3组患者的一般资料、手术麻醉时间和血浆白蛋白等经比较,组间差异均无统计学意义。中度贫血组患者的丙泊酚使用量低于轻度贫血组或正常组(P <0.05),且血红蛋白与丙泊酚用量之间呈弱相关(r=0.293,P=0.046)。结论在BIS监测下,中度贫血的妇科患者在实施腹腔镜手术时需降低丙泊酚用量。AIM To discuss the correlation between propofol dosage and hemoglobin in gynecological patients under general anesthesia by bispectral index(BIS)monitoring.METHODS A total of 48 patients,from 35 to 55 years old,ASAⅠorⅡ,BMI 18-24 kg·m^(-2),undergoing elective gynecological endoscopic surgery during genneral anesthesia were enrolled in this prospective study.Patients were divided into 3 groups based on preoperative hemoglobin level:normal hemoglobin group,mild anemia group and moderate anemia group,respectively.All patients used an unified anesthesia program,the general information and anesthesia time and propofol dosage of 3 groups were compared.RESULTS There were no significant differences in general information,anesthesia time,plasma albumin and so on among 3 groups.The dosage of propofol in moderate anemia group was lower than that in mild anemia group or normal hemoglobin group(P<0.05),and there was a weak correlation between hemoglobin and propofol dosage(r=0.293,P=0.046).CONCLUSION Under BIS monitoring,gynecological patients with moderate anemia need to reduce the dosage of propofol during laparoscopic surgery.
