床边纤维支气管镜引导下经鼻气管插管抢救呼吸衰竭的临床效果分析

摘　　要：目的:分析床边纤维支气管镜引导下经鼻气管插管抢救呼吸衰竭的临床效果。方法:选取2016年9月-2019年5月收治的74例呼吸衰竭患者,按照抽签的方式分为观察组和对照组,每组37例。对照组接受喉镜引导下经口气管插管治疗,观察组接受床边纤维支气管镜引导下经鼻气管插管治疗,对比两组患者的治疗效果、并发症发生率、生命体征、治疗前后血气指标。结果:观察组的治疗总有效率为94.59%,高于对照组的75.68%(P<0.05)。观察组并发症发生率为8.11%,低于对照组的27.03%(P<0.05)。观察组舒张压(82.68±6.21)mm Hg、收缩压(125.21±6.48)mm Hg、SpO_(2)(95.75±5.76)%、心率(103.01±5.31)次/min均优于对照组(P<0.05)。治疗后,观察组PaO_(2)(86.64±15.46)mm Hg、PaCO_(2)(42.58±12.15)mm Hg、pH(7.46±0.03)mm Hg,均优于对照组(P<0.05)。结论:床边纤维支气管镜引导下经鼻气管插管抢救呼吸衰竭的临床效果显著,能够帮助患者快速恢复正常通气状态,从而维持正常的生命状态。Objective:To analyze the clinical effect of bedside fiberoptic bronchoscope guided endotracheal intubation in rescuing respiratory failure.Method:A total of 74 patients with respiratory failure admitted to our hospital from September 2016 to May 2019 were enrolled in the study.The patients were given two different types of endotracheal intubation treatment,and the patients were divided into observation group and control group according to the drawing method,with 37 cases in each group.The control group received oral tracheal intubation guided by laryngoscope,and the observation group received nasal endotracheal intubation under the guidance of bedside fiberoptic bronchoscope.The treatment effect,incidence of complications,vital signs and blood gas indexes before and after treatment were compared between the two groups.Result:The total effective rate of the observation group was 94.59%,which was higher than 75.68%of the control group(P<0.05).The complication rate of observation group was 8.11%,which was lower than 27.03%of control group(P<0.05).The diastolic pressure of(82.68±6.21)mm Hg,systolic pressure of(125.21±6.48)mm Hg,SpO_(2) of(95.75±5.76)%and heart rate of(103.01±5.31)times/min in the observation group were all better than those in the control group(P<0.05).After treatment,PaO_(2) of(86.64±15.46)mm Hg,PaCO_(2) of(42.58±12.15)mm Hg,pH of(7.46±0.03)mm Hg in the observation group were better than those in the control group(P<0.05).Conclusion:The clinical effect of bedside fiberoptic bronchoscope guided nasotracheal intubation in rescuing respiratory failure is significant,which can help patients quickly recover to normal ventilation state and maintain normal life state.
