摘  要:目的:探究DECT(双能量CT)对痛风尿酸盐结晶的检测价值,探究DECT对痛风的诊断价值。方法:本研究在2019年3月-2020年9月于笔者所在医院就诊的患者中,以随机法抽选出65例痛风确诊者作为研究组。同时选取35例非痛风者作为对照组,对两组患者均利用DECT进行检查,处理、分析检查结果,统计并计算DECT成像检查尿酸盐结晶对痛风诊断的灵敏度、特异性、阳性预测值及阴性预测值。同时对两组患者均进行血尿酸水平的检查,比较两组血尿酸水平。结果:组间血尿酸水平差异有统计学意义(P<0.05);利用DECT对尿酸盐结晶进行检测诊断痛风的灵敏度为92.31%,特异性为94.29%,阳性预测值为96.77%,阴性预测值为86.84%。结论:在痛风的临床诊断中,利用DECT成像检测尿酸盐结晶有着较高的特异度及敏感度,可作为临床痛风患者筛查中的重要手段。Objective:To explore the DECT(dual energy CT)detection value of gout urate crystals,and the diagnostic value of DECT for gout.Method:From March 2019 to September 2020 in our hospital,by the method of random this research selected out of 65 cases of gout diagnosed as a team.Selected 35 cases without gout as a control at the same time,the inspection on two groups of patients used the DECT.Processed and analysed test results,calculated the DECT imaging urate crystals of gout diagnosed sensitivity,specificity,positive predictive value and negative predictive value.Checked and compared the two groups of patients’blood uric acid level.Result:There was no difference between two groups of blood uric acid level(P<0.05).Detection and diagnosis of gout by urate crystals using DECT,sensitivity was 92.31%,specificity was 94.29%,positive predictive value was 96.77%,negative predictive value was 86.84%.Conclusion:In clinical diagnosis of gout,using DECT imaging detection of urate crystals have higher specific and sensitivity,can be used as a clinical screening of the important approaches for patients with gout.
