摘　　要：目的探究血液透析血管通路护理小组模式对血液透析患者负面情绪、血管通路循环和配合度的影响。方法选取苏州市立医院2017年5月2019年5月收治的血液透析患者80例,随机分人对照组(n=40)和观察组(n=40)。对照组患者实施常规护理,观察组患者在常规护理的基础上实施血液透析血管通路护理小组模式干预。观察两组患者的负面情绪改善情况、生活质量、血管通路再循环率、穿刺配合度和并发症发生率。结果干预后,两组抑郁自评量表(SDS)评分和焦虑自评量表(SAS)评分均显著低于干预前(P值均<0.05),且观察组评分均显著低于对照组(P值均<0.05)。干预后,两组生活质量评定表(SF-36)各项评分均显著高于干预前(P值均<0.05),且观察组的评分均显著高于对照组(P值均<0.05)。观察组并发症发生率显著低于对照组(P<0.05)。观察组血管通路再循环率为15.00%,显著低于对照组的35%(P<0.05)。观察组穿刺配合度评分显著高于对照组(P<0.05)。结论血液透析血管通路护理小组模式可改善血液透析患者负面情绪,降低血管通路再循环率,提高生活质量和穿刺配合度,减少并发症发生率,推荐临床推广使用。Objective To explore the effect of team nursing model in hemodialysis vascular access on negative emotions,vascular access circulation and coordination of hemodialysis patients.Methods 80 patients receiving hemodialysis in Suzhou Municipal Hospital from May 2017 to May 2019 were selected and randomly divided into a control group(n=40)and an observation group(n=40).The control group was given routine nursing and the observation group was intervened by team nursing model of hemodialysis vascular access on the basis of routine nursing.The improvement of negative emotions,quality of life,rate of vascular access recirculation,puncture coordination and complication rate were observed.Results After the intervention,the scores of Self_Rating Depression Scale(SDS)and Self-Rating Anxiety Scale(SAS)of the two groups were significantly lower than those before the intervention(both P<0.05)and the scores.in the observation group were significantly lower than those in the control group(both P<0.05)..The scores of 36-Item Short Form Health Survey(SF-36)in the two groups after intervention were significantly higher than those before intervention(all P<0.05)and the observation group was significantly higher than the control group(all P<0.05).The complication rate of the observation group was significantly lower than that of the control group(P<0.05).The vascular access recirculation rate of the observation group was 15.00%,which was significantly lower than 35% of the control group(P<0.05).The puncture cooperation score of the observation group was significantly higher than that of the control group(P<0.05).Conclusion The team nursing model in hemodialysis vascular access can effectively improve the negative emotions of patients receiving hemodialysis,reduce the rate of vascular access recirculation,improve the quality of life and puncture coordination and reduce the incidence of complications.It is recommended for clinical promotion.
