经翼点入路与经眶上外侧入路颅内前循环动脉瘤夹闭术的疗效比较

摘　　要：目的探究经翼点入路与经眶上外侧入路颅内前循环动脉瘤夹闭术的疗效。方法50例颅内前循环动脉瘤患者,以随机数字表法分为实验组与对照组,各25例。两组均采用颅内前循环动脉瘤夹闭术,对照组采用经翼点入路方式进行治疗,实验组采用经眶上外侧入路方式进行治疗。对比两组手术指标、并发症发生情况及手术前后血清神经元特异性烯醇化酶(NSE)、S100β水平。结果实验组切口长度、术中出血量、手术时长、骨窗范围、骨质损伤范围均显著优于对照组,差异具有统计学意义(P<0.05)。实验组并发症发生率为4.00%显著低于对照组的32.00%,差异具有统计学意义(P<0.05)。术后,实验组NSE、S100β水平分别为(50.37±11.96)μg/L、(0.64±0.03)μg/L,显著低于对照组的(61.83±10.19)μg/L、(0.75±0.06)μg/L,差异具有统计学意义(P<0.05)。结论对颅内前循环动脉瘤夹闭术患者应用经眶上外侧入路方式治疗,可有效优化患者的临床指标,降低术后并发症发生率,减少手术产生的脑损伤,治疗效果显著,值得推广。Objective To investigate the efficacy of clipping of intracranial anterior circulation aneurysms via pterional approach and lateral supraorbital approach.Methods A total of 50 patients with intracranial anterior circulation aneurysm were divided into experimental group and control group according to random numerical table,with 25 cases in each group.Both groups underwent clipping of intracranial anterior circulation aneurysms.The control group was treated with pterional approach,and the experimental group was treated with lateral supraorbital approach.The surgical indicators,occurrence of complications,serum neuron specific enolase(NSE)and S100β before and after operation of the two groups were compared.Results The incision length,intraoperative blood loss,operation time,bone window range,bone damage range of the experimental group were significantly better than those of the control group,and the difference was statistically significant(P<0.05).The incidence of complications 4.00%of the experimental group was significantly lower than 32.00%of the control group,and the difference was statistically significant(P<0.05).After operation,the NSE and S100βof the experimental group were(50.37±11.96)and(0.64±0.03)μg/L,which were significantly lower than(61.83±10.19)and(0.75±0.06)μg/L of the control group,and the difference was statistically significant(P<0.05).Conclusion Clipping of intracranial anterior circulation aneurysms via lateral supraorbital approach has remarkable effect,which can effectively optimize the clinical indicators of patients,lower the incidence of postoperative complications,and reduce the brain injury caused by surgery.It is worthy of promotion.
