摘 要:目的研究补充维生素D对慢性阻塞性肺疾病(COPD)患者肺功能及血气指标的影响。方法60例COPD患者,随机分为对照组与研究组,每组30例。对照组患者行常规治疗,研究组患者在常规治疗基础上补充维生素D。比较两组患者治疗前后1,25-二羟基维生素D_(3)[1,25(OH)_(2)D_(3)]水平及肺功能指标[第1秒用力呼气容积(FEV1)、第1秒用力呼气容积占预计值的百分比(FEV1%)、第1秒用力呼气容积占用力肺活量的百分比(FEV1/FVC)]、血气指标[动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))、动脉血氧饱和度(SaO_(2))]。结果治疗后,研究组患者1,25(OH)_(2)D_(3)水平及FEV1、FEV1%、FEV1/FVC分别为(31.2±2.3)ng/ml、(2.87±0.28)L、(81.69±12.47)%、(86.18±3.69)%,均高于对照组的(17.4±4.3)ng/ml、(2.53±0.16)L、(69.33±5.48)%、(76.42±8.19)%,差异具有统计学意义(P<0.05)。治疗后,研究组患者PaO_(2)、SaO_(2)均高于对照组,差异具有统计学意义(P<0.05);两组患者PaCO_(2)比较差异无统计学意义(P>0.05)。结论补充维生素D能显著改善COPD患者的肺功能指标和血气指标,改善患者临床症状。Objective To study the effect of vitamin D supplementation on pulmonary function and blood gas indexes in patients with chronic obstructive pulmonary disease(COPD).Methods A total of 60 COPD patients were randomly divided into control group and research group,with 30 cases in each group.The control group was treated with conventional therapy,and the research group was treated with vitamin D supplementation on the basis of conventional therapy.The 1,25-dihydroxyvitamin D_(3)[1,25(OH)_(2)D_(3)]levels and lung function indicators[forced expiratory volume in the first second(FEV1),forced expiratory volume in the first second as a percentage of the predicted value(FEV1%),FEV1/forced vital capacity(FEV1/FVC)],blood gas indicators[arterial oxygen partial pressure(PaO_(2)),arterial carbon dioxide partial pressure(PaCO_(2)),arterial oxygen saturation(SaO_(2)))]before and after treatment were compared between the two groups.Results After treatment,the 1,25(OH)_(2)D_(3),FEV1,FEV1%,FEV1/FVC of the research group were(31.2±2.3)ng/ml,(2.87±0.28)L,(81.69±12.47)%,(86.18±3.69)%,which were higher than(17.4±4.3)ng/ml,(2.53±0.16)L,(69.33±5.48)%,(76.42±8.19)%of the control group,and the difference was statistically significant(P<0.05).After treatment,the PaO_(2) and SaO_(2) of the research group were higher than those of the control group,and the difference was statistically significant(P<0.05).There was no statistically significant difference in PaCO_(2) between the two groups(P>0.05).Conclusion Vitamin D supplementation can significantly improve the pulmonary function and blood gas indexes of COPD patients,and improve their clinical symptoms.
