体位复位经皮椎体成形术结合鲑降钙素皮下注射治疗老年骨质疏松性胸腰椎压缩性骨折的疗效分析

摘　　要：目的研究老年骨质疏松性胸腰椎压缩性骨折(OVCF)患者接受体位复位经皮椎体成形术(PVP)结合鲑降钙素皮下注射治疗的临床效果。方法58例老年OVCF患者,按照随机数字表法分为研究组和对照组,各29例。研究组患者接受体位复位PVP结合鲑降钙素皮下注射方案治疗,对照组患者仅接受体位复位PVP治疗。对比两组患者术前和术后6个月视觉模拟评分法(VAS)评分、Barthel指数、日本骨科协会(JOA)评分。结果两组术前的VAS评分对比,差异无统计学意义(P>0.05)。术后6个月,两组的VAS评分均明显低于术前,差异具有统计学意义(P<0.05);且研究组VAS评分(0.98±0.31)分明显低于对照组的(3.63±0.45)分,差异具有统计学意义(P<0.05)。两组术前的Barthel指数对比,差异无统计学意义(P>0.05);术后6个月,两组的Barthel指数均明显高于术前,且研究组明显高于对照组,差异具有统计学意义(P<0.05)。两组术前的JOA评分对比,差异无统计学意义(P>0.05);术后6个月,两组的JOA评分均明显高于术前,且研究组明显高于对照组,差异具有统计学意义(P<0.05)。结论老年OVCF患者应用体位复位PVP联合鲑降钙素皮下注射方案治疗,并根据个体需要适量补充钙剂和维生素D,能够有效缓解患者的骨折部位疼痛,改善患者的腰椎功能和日常活动能力,提高预后质量,临床应用价值突出。Objective To study the clinical effect of postural reduction and percutaneous vertebroplasty(PVP)combined with subcutaneous injection of salmon calcitonin in the treatment of senile osteoporotic vertebral compression fracture(OVCF).Methods A total of 58 senile patients with OVCF were divided into research group and control group according to random numerical table,with 29 cases in each group.The research group was treated with postural reduction and PVP combined with subcutaneous injection,and the control group was treated with postural reduction and PVP.The visual analogue scale(VAS)score,Barthel index and Japan Orthopedic Association(JOA)score before operation and 6 months after operation were compared between the two groups.Results Before operation,there was no statistically significant difference in VAS score between the two groups(P>0.05).6 months after operation,the VAS score of the two groups were obviously lower than those before operation,and the difference was statistically significant(P<0.05).The VAS score(0.98±0.31)points of the research group was obviously lower than(3.63±0.45)points of the control group,and the difference was statistically significant(P<0.05).Before operation,there was no statistically significant difference in Barthel index between the two groups(P>0.05).6 months after operation,the Barthel index of the two groups were obviously higher than those before operation,and the research group was obviously higher than the control group.All the difference was statistically significant(P<0.05).Before operation,there was no statistically significant difference in JOA score between the two groups(P>0.05).6 months after operation,the JOA score of the two groups were obviously higher than those before operation,and the research group was obviously higher than the control group.All the difference was statistically significant(P<0.05).Conclusion For senile patients with OVCF,postural reduction and PVP combined with subcutaneous injection of salmon calcitonin,and appropriate supplement of calciu
