大隐静脉曲张高位结扎+腔内激光闭合术治疗原发性下肢静脉曲张的临床体会

摘　　要：目的研究大隐静脉高位结扎+腔内激光闭合术(EVLT)治疗原发性下肢静脉曲张的临床效果。方法100例原发性下肢静脉曲张患者,随机分为对照组和观察组,各50例。对照组采用大隐静脉高位结扎+抽剥术治疗,观察组采用大隐静脉高位结扎+腔内激光闭合术治疗。比较两组手术指标、术后疼痛评分、术后并发症发生情况。结果观察组术中出血量(5.3±0.6)ml、手术时间(35.3±3.6)min、住院时间(3.5±1.8)d均显著少于对照组的(15.8±1.8)ml、(55.7±7.8)min、(8.6±2.4)d,差异具有统计学意义(P<0.05)。观察组术后疼痛评分(2.5±1.8)分显著低于对照组的(5.5±1.3)分,差异具有统计学意义(t=9.554,P<0.05)。术后早期,两组均未出现明显的急性肺动脉栓塞、急性深静脉血栓等严重并发症。结论大隐静脉高位结扎+腔内激光闭合术用于原发性下肢静脉曲张治疗,具有创伤小、术后恢复快、并发症少等优势。Objective To study the clinical effect of high ligation of great saphenous varicose veins and endovenous laser treatment(EVLT)in the treatment of primary varicose veins of lower extremities.Methods A total of 100 patients with primary varicose veins of lower extremities were randomly divided into control group and observation group,with 50 cases in each group.The control group was treated with high ligation of great saphenous varicose veins and stripping,and the observation group was treated with high ligation of great saphenous varicose veins and endovenous laser treatment.The surgical indicators,postoperative pain score and occurrence of postoperative complications were compared between the two groups.Results The intraoperative blood loss(5.3±0.6)ml,operation time(35.3±3.6)min and hospitalization time(3.5±1.8)d of the observation group were significantly less than(15.8±1.8)ml,(55.7±7.8)min and(8.6±2.4)d of the control group,and the difference was statistically significant(P<0.05).The postoperative pain score(2.5±1.8)points was significantly lower than(5.5±1.3)points of the control group,and the difference was statistically significant(t=9.554,P<0.05).There were no severe complications such as acute pulmonary embolism and acute deep vein thrombosis in both groups.Conclusion High ligation of great saphenous vein and endovenous laser treatment have the advantages of small trauma,faster recovery after surgery,and fewer complications for primary varicose veins of lower extremities.
