腔镜辅助下环乳晕小切口超声刀碎吸法治疗重度男性乳房发育症的临床分析

摘　　要：目的探讨腔镜辅助下环乳晕小切口超声刀碎吸法治疗重度男性乳房发育症(GYN)的疗效。方法回顾性分析16例采用局部肿胀麻醉技术行腔镜辅助下环乳晕小切口超声刀碎吸法治疗重度GYN患者的临床资料。结果16例重度GYN患者的乳房切除增生的乳腺组织约80~162 g,平均(112±20.65)g,腺体型12例,脂肪型4例。大体标本外观为:扁平肿块,盘状,质韧,无完整包膜,切面灰白。本组病例切下的标本均符合上述外观,直径最大的达18 cm为脂肪型。16例患者仅有2例切口轻度感染,其余均一期愈合;无皮下积液,无皮肤及乳头坏死。随访6~24个月,术区外观良好,无复发表现,效果满意。结论在局部肿胀麻醉技术下行腔镜辅助下环乳晕小切口超声刀碎吸法切除增生乳腺,是一种治疗重度GYN的良好方法,手术操作简单、安全、损伤轻、出血少、恢复快。Objective To discuss the efficacy of endoscopic assisted small circum-areola incision ultrasonic knife aspiration in the treatment of severe gynecomastia(GYN).Methods The clinical data of 16 patients with severe gynecomastia who were treated with local tumescent anesthesia by endoscopic assisted small circum-areola incision ultrasonic knife aspiration were retrospectively analyzed.Results In 16 patients with severe GYN,the hyperplasia of breast tissue was about 80-162 g,with an average of(112±20.65)g.There were 12 cases of glandular type and 4 cases of fatty type.Appearance of gross specimen:flat mass,discshaped,tough texture,no intact capsule,gray-white cut surface.All the cut specimens conformed to the above appearances,and the largest diameter of 18 cm was fat type.In 16 patients,only 2 cases had mild wound infection,and the other incisions were primary healing;there was no subcutaneous effusion,skin and nipple necrosis.All patients were followed up for 6-24 months,the appearance of the operation area was good,there was no recurrence,and the effect was satisfactory.Conclusion It is a good method to treat severe GYN with local tumescent anesthesia by endoscopic assisted small circum-areola incision ultrasonic knife aspiration.The operation is simple,safe,with less damage,less bleeding,and quick recovery.
