乳腺组织瓣法矫治乳房下垂的临床应用

摘　　要：目的探讨乳腺组织瓣法在乳房下垂的临床效果。方法在乳晕周切除内外环间全层皮肤,然后在乳房中下设计并切取舌状乳腺组织瓣,将此瓣向乳腺后上方旋转提升固定于胸肌筋膜上,矫治乳房下垂。结果该组30例患者切口均Ⅰ期愈合,无乳头乳晕坏死。所有患者术后经3~24个月随访,乳晕周围皮肤瘢痕不明显,乳房下垂得到有效矫正,外形满意。结论乳腺组织瓣法矫治乳房下垂,乳房形态自然挺拔,效果满意。Objective To explore the clinical effect of double-loop breast tissue flap on breast sagging.Methods Using double ring incision around the areola,to remove skin between inner and outer rings and completely separated under the skin,and then to cut the tongue glandular tissue flap in the lower middle of the breast and rotate this flap back and up to increase areola area.Suturing the glands on both sides,and then cutting the glands vertically at the outer ring edge to the superficial fascia layer and separate,to form lateral glandular flap.Rotating and lifting this flap to fix it on the inner and upper pectoral fascia to further shrink the base of the breast.Result 30 patients in this group were healed.No case had hematoma,infection,nipple or areola necrosis.All patients were followed up for 3 to 24 months after operation.Skin scars around areola were not obvious.All had soft texture,blood flow and were feeling good.Breast sagging was effectively corrected,and it had satisfactory appearance.Conclusion Treatment of sagging breast with double-loop breast tissue flap has scientific design with simple operation.The method can reshape the breast's good shape and cause satisfactory effect.
