自体伸肌支持带修复腕部指伸肌腱自发性断裂的临床分析

摘　　要：目的探讨自体伸肌支持带用于腕部指伸肌腱自发性断裂修补后防止局部粘连的临床效果。方法对15例自发性手指伸肌腱断裂的患者,根据肌腱损伤情况切取大小不等的伸肌支持带,吻合肌腱后,将伸肌支持带于吻合端进行缝合覆盖,术后给与石膏支具固定3周。结果术后局部滑膜和肌腱组织送病理检查均显示为慢性炎症伴局灶性坏死。术后随访6~36个月,未见肌腱再次断裂及局部粘连,优良率为100%。结论对于手指伸肌腱自发性断裂,采用伸肌支持带局部覆盖吻合端对预防肌腱粘连疗效满意。Objective To investigate the clinical effect of autogenous extensor band on preventing local adhesion after repairing the spontaneous rupture of extensor tendon of wrist.Methods 15 cases of spontaneous rupture of extensor tendon of finger were treated,the retinaculum of extensor retinaculum was covered with anastomotic end.The injured tendon was repaired and fixed with plaster brace for 3 weeks after operation.Results Pathological examination of local synovium and tendon showed chronic inflammation with focal necrosis.Followup for 6-36 months showed no re-rupture of tendon and local adhesion.The excellent and good rate was 100%.Conclusion For the spontaneous rupture of extensor tendon of fingers,the local coverage of the anastomotic end with extensor retinaculum is satisfactory for the prevention of tendon adhesion.
