摘　　要：目的:探讨鹊炎灵方口服加灌肠联合宫腹腔镜治疗子宫内膜异位症(EMS)对受孕率及血清糖类抗原125(CA125)、抗子宫内膜抗体(EmAb)、糖类抗原199(CA199)的影响。方法:选择2018年7月~2020年7月于本院就诊的180例EMS患者作为研究对象,采用随机数字表法将其分为观察组(n=90)和对照组(n=90)。对照组采用腹腔镜手术+散结镇痛胶囊治疗,观察组采用腹腔镜手术+鹊炎灵方口服+灌肠治疗治疗,均治疗3个月,并随访6个月。比较两组治疗后的疗效,治疗前后的CA125、EmAb、CA199、血清炎症细胞因子[白介素-6(IL-6)、白介素-8(IL-18)、肿瘤坏死因子-α(TNF-α)]及性激素[卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E2)、孕酮(P)]水平,统计随访后的复发率、受孕率以及治疗期间的不良反应发生率。结果:治疗后,观察组的治疗总有效率93.33%明显高于对照组的83.33%;同时,观察组患者血清CA125、EmAb、CA199、IL-6、IL-18和TNF-α水平均低于对照组;性激素指标FSH、LH、E2和P水平低于对照组。随访期间,观察组患者的复发率低于对照组,受孕率明显高于对照组;治疗期间,观察组不良反应总发生率明显低于对照组。结论:采用鹊炎灵方口服加灌肠联合腹腔镜治疗EMS,可降低血清CA125、EmAb、CA199水平,减轻患者机体炎症反应,改善性激素水平,减少术后复发并提高受孕率,具有较好的临床疗效。Objective To explore the effects of oral Queyanling Decoction combined with enema and laparoscopy on pregnancy rate,serum carbohydrate antigen 125(CA125),anti-endometrial antibody(EmAb)and carbohydrate antigen 199(CA199)in endometriosis(EMS)patients.Methods A total of 180 EMS patients treated in the hospital from July 2018 to July 2020 were enrolled as the research objects.They were divided into observation group(n=90)and control group(n=90)by random number table method.The control group was treated with laparoscopy and Sanjiezhentong capsules,while observation group was treated with laparoscopy,oral Queyanling Decoction and enema.Both groups were treated for 3 months and followed up for 6 months.The curative effect after treatment,levels of CA125,Emab,CA199,serum inflammatory cytokines[interleukin-6(IL-6),interleukin-8(IL-18),tumor necrosis factor-α(TNF-α)]and sex hormones[follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E2),progesterone(P)]before and after treatment were compared between the two groups.The recurrence rate after follow-up,pregnancy rate,and incidence of adverse reactions during treatment were statistically analyzed.Results After treatment,the total response rate of treatment in observation group was significantly higher than that in control group(93.33%vs 83.33%),levels of serum CA125,Emab,CA199,IL-6,IL-18 and TNF-αwere lower than those in control group,and levels of sex hormone indexes(FSH,LH,E2,P)were lower than those in control group.The recurrence rate during follow-up in observation group was lower than that in control group,while pregnancy rate was significantly higher than that in control group.During treatment,the total incidence of adverse reactions in observation group was significantly lower than that in control group.Conclusion The application of oral Queyanling Decoction combined with enema and laparoscopy in the treatment of EMS can reduce levels of serum CA125,Emab and CA199,alleviate body inflammation response,improve levels of sex hormones,reduce postoperat
