摘　　要：目的:探讨颅内外血管病变对BPPV复发的影响。方法:选取经神经内科门诊确诊的150例BPPV患者,随访1年,并将其分为复发组和无复发组。采用颈动脉彩超检测患者颈动脉附壁斑块、头颅核磁共振检查脑部腔隙梗死灶、评估大脑白质变性分数;血液生化仪检测血清中总胆固醇、低密度脂蛋白、甘油三脂、同型半胱胺酸的含量;糖化血红蛋白仪测定血浆中的糖化血红蛋白;比较两组患者上述指标有无统计学差异。结果:BPPV复发组患者中并发心脑血管病的比例以及颈动脉附壁斑块的阳性检出率相比于无复发组患者显著增高。结论:在中老年患者中,并发心脑血管病、颈动脉附壁斑块是BPPV复发的危险因素。Objective To investigate the effect of intracranial or extracranial vascular lesions on BPPV recurrence.Methods Carotid artery mural plaques were detected by color doppler ultrasound,cerebral lacunar infarcts were examined by cranial magnetic resonance imaging,and white matter degeneration scores were evaluated.The concentrations of total cholesterol,low density lipoprotein,triglyceride and homocysteine in serum were detected by blood biochemical instrument,respectively.HbIC in plasma was detected by HbA1c analyzer.Comparison of the above examination results between the two groups.Results The ratio of cardiovascular and cerebrovascular disease and the positive detection ratio of carotid artery mural plaques in patients with BPPV recurrence group were higher than those without recurrence group significantly.Conclusion In middle-aged and elderly BPPV patients who with complicating cardiovascular and cerebrovascular disease and carotid artery mural plaques are risk factors for BPPV recurrence.
