摘　　要：目的:探讨加味桃核承气汤对脓毒症胃肠功能障碍患者肠胃动力、免疫指标及肝肾功能的影响。方法:将114例脓毒症胃肠功能障碍患者随机分为治疗组和对照组。对照组57例采用常规西医疗法,治疗组在此基础上结合中医加味桃核承气汤治疗,疗程均为1周。比较两组临床疗效及肠胃动力指标、免疫指标、肝肾功能指标。结果:相较于治疗前,两组治疗后G17、PGⅠ、PGⅠ/PGⅡ和CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)水平均有明显升高,且治疗组比对照组升高程度更为显著;两组治疗后中医证候积分和肝肾功能指标均有明显降低,且治疗组比对照组降低程度更为显著。治疗组临床疗效优于对照组。结论:加味桃核承气汤能够提升脓毒症胃肠功能障碍患者整体疗效,其作用机制与促进胃肠动力恢复,提高免疫功能和肝肾功能有关。Objective To explore the effect of Modified Taohe Chengqi Decoction on gastrointestinal motility,immune index and liver and kidney function in patients with sepsis and gastrointestinal dysfunction.Methods 114 patients with sepsis gastrointestinal dysfunction were randomly divided into treatment group and control group.57 cases in the control group were treated with conventional western medicine.On this basis,the treatment group was treated with Taohe Chengqi Decoction.The course of treatment was one week.The clinical effect,gastrointestinal motility index,immune index,liver and kidney function index were compared between the two groups.Results Compared with before treatment,the levels of G17,PGⅠ,PGⅠ/PGⅡ and CD3^(+),CD4^(+),CD4^(+)/CD8^(+) in the two groups were significantly increased after treatment,and the increase degree of the treatment group was more significant than that of the control group;the TCM syndrome score and liver and kidney function indexes of the two groups were significantly decreased after treatment,and the decrease degree of the treatment group was more than that of the control group.The clinical effect of the treatment group was better than that of the control group.Conclusion Modified Taohe Chengqi Decoction can improve the overall efficacy of patients with sepsis gastrointestinal dysfunction,and its mechanism is related to promoting the recovery of gastrointestinal motility,improving immune function and liver and kidney function.
