摘 要:目的:探究多发肋骨骨折内固定对急性胸部创伤患者呼吸功能的影响。方法:选取本院2017年1月~2020年7月收治的具有连续多根肋骨骨折的患者436例分为观察组和对照组。其中观察组患者实施内固定治疗,对照组则实施保守方式治疗。采用肺功能检测仪检测患者肺功能相关指标(VC、FEV1及PEF),患者机体的疼痛指数通过专人依照视觉模拟评分法进行评分和记录,并做AIS-ISS创伤评分。结果:两组患者VAS评分、肺功能指标(VC、FEV1及PEF)比较,差异有统计学意义;AIS-ISS创伤评分差异无统计学意义。患者通过内固定治疗后,疼痛明显减轻,肺功能指标(VC、FEV1及PEF)得到改善。VC与ISS评分、疼痛指数、肺损伤程度具有负相关性。结论:多发肋骨骨折内固定可有效减轻急性胸部创伤患者胸痛和肺功能损伤,有效改善患者的呼吸功能。Objective To investigate the effect of multiple rib fracture internal fixation on respiratory function in patients with acute chest trauma.Methods 436 patients with continuous multiple rib fractures admitted to our hospital from January 2017 to July 2020 were selected and divided into observation group and control group on average.The observation group received internal fixation treatment,while the control group received conservative treatment.Pulmonary function detector was used to detect the lung function related indicators(VC,FEV1 and PEF)of the patients.The pain index of the patients was scored and recorded by a specially assigned person according to the visual simulation scoring method,and the AIS-ISS trauma score was performed.Results There was significant difference in VAS score and pulmonary function index(VC,FEV1 and PEF)between the two groups,but there was no significant difference in AIS-ISS trauma score.After internal fixation,the pain was significantly reduced and lung function indexes(VC,FEV1 and PEF)were improved.VC was negatively correlated with ISS score,pain index and lung injury.Conclusion Internal fixation of multiple rib fractures can effectively reduce chest pain and lung function injury in patients with acute chest trauma,and effectively improve the respiratory function of patients.
