实用新型皮管训练器的临床应用及疗效

摘　　要：目的介绍一种实用新型皮管训练器并探讨其在临床的应用价值。方法对自2019年5月至2020年8月在我院行带蒂腹股沟皮瓣转移术的17例患者使用该新型皮管训练器进行规范的皮瓣皮管训练,评估护士熟练掌握该新型训练器所需的时间,皮管训练处皮肤破溃及皮瓣感染的发生率,皮瓣断蒂时间,断蒂后皮瓣的成活率以及患者对护理的总体满意率等指标。结果护士熟练掌握该新型皮管训练器的时间为5~12 min,平均7.37 min。17例患者中仅1例出现皮管训练处轻度皮肤破溃及少量渗出,发生率为5.88%(1/17),但未造成皮瓣感染。所有患者均在接受带蒂腹股沟皮瓣转移术后21~24 d(平均21.3 d)行皮瓣断蒂术,所有皮瓣在断蒂后均完全存活。患者对护理的整体满意度为94.12%(16/17)。结论使用实用新型皮管训练器对带蒂腹股沟皮瓣术后患者进行皮管训练具有较高的临床有效率,相关并发症少、患者认可度高,且护士易于掌握操作方法,值得在临床推广。Objective To introduce a new type of pedicled flap skin tube training device and discuss its clinical application value.Methods From May 2019 to August 2020,17 patients with pedicled groin flap transfer in our hospital were trained with a new type of skin tube training device.The time required for nurses to master the new type of training device,the incidence of skin ulceration and skin flap infection at the skin tube training place,the time of flap pedicle cut-off,the survival rate of flap after pedicle cut-off,and the overall satisfaction rate of patients with nursing care were evaluated.Results The time for nurses to master the new skin tube training device was 5 to 12 minutes,with an average of 7.37 minutes.In 17 patients,only 1 patient had mild skin ulceration and a small amount of exudation at the skin tube training site,with the incidence rate being 5.88%(1/17),but no skin flap infection was caused.All the patients underwent pedicle cut-off operation 21 to 24 days with an average of 21.3 days after pedicled groin flap transfer.All the flaps survived completely after pedicle cut-off.The overall satisfaction of the patients with nursing was 94.12%(16/17).Conclusion The use of the new type of pedicled flap skin tube training device has higher clinical efficiency,less related complications,high recognition of patients,and nurses are easy to master the operation method,which is worthy of clinical promotion.
