摘 要:目的:研究中西药结合(天麻素联合复合辅酶)抢救重度一氧化碳(CO)中毒的疗效。方法:2016年1月-2019年12月收治重度CO中毒患者111例,随机分为对照组57例和试验组54例。对照组采用ATP+辅酶A+胞磷胆碱治疗;试验组采用天麻素联合复合辅酶治疗。比较两组治疗效果。结果:试验组治疗后神经功能缺损评分低于对照组,Barthel评分高于对照组,差异有统计学意义(P<0.05);试验组治疗后肌酸激酶MB同工酶(CKMB)、肌钙蛋白、脑钠肽(BNP)、C反应蛋白(hs-CRP)等指标水平均低于对照组,左心射血分数水平高于对照组,差异有统计学意义(P<0.05);试验组肌功能减退发生率显著低于对照组,差异有统计学意义(P<0.05)。结论:天麻素联合复合辅酶抢救重度CO中毒的效果良好,有助于改善神经功能,促进神经系统恢复,促进心脏功能恢复,减轻心肌细胞损伤。Objective:To study the curative effect of combination of Chinese and Western medicine(gastrodin combined with compound coenzyme)in rescuing severe CO poisoning.Methods:From January 2016 to December 2019,111 cases of patients with severe CO poisoning were enrolled,they were randomly divided into two groups with 57 cases in control group and 54 cases in experimental group.The control group was treated with ATP+coenzyme A+citicoline.The experimental group was treated with gastrodin combined with composite coenzyme.The therapeutic effects between the two groups was compared.Results:The score of neurological deficit after treatment in the experimental group was lower than that in the control group,the Barthel score was higher than the control group,the differences were statistically significant(P<0.05).After treatment,the levels of CKMB,troponin,BNP and C-reactive protein(hs-CRP)in the experimental group were lower than those in the control group,the left ventricular ejection fraction was higher than control group,the differences were statistically significant(P<0.05).The incidence of muscular dysfunction in the experimental group was significantly lower than that in the control group,the difference was statistically significant(P<0.05).Conclusion:Gastrodin combined with composite coenzyme is effective in rescuing severe CO poisoning,which is helpful to improve nerve function,promote nervous system recovery,promote cardiac function recovery and alleviate myocardial cell injury.
