玉屏风颗粒合四物颗粒对慢性阻塞性肺疾病稳定期患者炎症因子及肺功能的影响

摘　　要：目的:探讨玉屏风颗粒合四物颗粒对慢性阻塞性肺疾病(COPD)稳定期患者炎症因子及肺功能的影响。方法:选取潍坊市益都中心医院2020年3月至2020年12月期间收治的60例COPD稳定期患者,随机分为观察组及对照组,各30例。两组患者均给予噻托溴铵治疗,观察组在上述治疗的基础上应用玉屏风颗粒合四物颗粒。观察两组患者肿瘤坏死因子(TNF–α)、白细胞介素–6(IL–6)、IL–10,比较两组患者治疗疗效,中医证候积分、肺功能指标〔1 s用力呼气容积(FEV1)、1 s用力呼气容积占用力肺活量比值(FEV1/FVC)〕变化。结果:两组患者治疗前TNF–α、IL–6及IL–10水平比较,差异无统计学意义(P>0.05)。治疗后第4周、第12周两组患者TNF–α、IL–6水平较治疗前降低,IL–10水平较治疗前升高,差异具有统计学意义(P<0.05)。治疗后观察组患者第4周、第12周TNF–α、IL–6水平低于对照组,差异具有统计学意义(P<0.05),但观察组IL–10水平与对照组比较,差异无统计学意义(P>0.05);治疗前两组患者的FEV1、FEV1/FVC水平及中医证候积分比较,差异无统计学意义(P>0.05)。治疗第12周后两组患者的FEV1、FEV1/FVC水平及中医证候积分均有不同程度改善,且观察组的FEV1、FEV1/FVC水平及中医证候积分均优于对照组,差异具有统计学意义(P<0.05)。结论:玉屏风颗粒合四物颗粒可减轻COPD稳定期患者机体炎症反应,改善肺功能。Objective To explore the effect of Yupingfeng granule combined with Siwu granule on serum inflammatory factors and pulmonary function in patients with stable chronic obstructive pulmonary disease (COPD).Method A total of 60 patients with stable COPD admitted into the hospital from March 2020-December 2020 were enrolled in the study and were randomly divided into two groups (observation group with 30 cases and control group with 30 cases).The two groups of patients were treated with tiotropium bromide.On this basis,the observation group was treated with Yupingfeng granule and Siwu granule.Two groups were treated for a period of 3 months.The levels of indexes of the lung function[the first second forced expiratory volume (FEV1),the ratio of FEV1 to FVC(FEV1/FVC)]and the concentrations of cytokines in serum (TNF-α、IL-6、IL-10) were detected before and after treatment respectively.The changes of lung function and serum inflammatory factors were compared between the two groups.Results After treatment for 3 months,the concentrations of TNF-α and IL-6 in both groups were significantly decreased(P < 0.05),and the concentrations of IL-10 in both groups were significantly increased in the two groups (P < 0.05).The concentrations of TNF-α and IL-6 in observation group were significantly lower than those in control group (P < 0.05),the concentration of IL-10 inobservation group was higher than that in control group (P > 0.05).After treatment,the levels of FEV1 and FEV1/FVC in both groups were significantly increased (P < 0.05),and the levels of FEV1 and FEV1/FVC in observation group was significantly higher than those in control group (P < 0.05).After treatment,the scores of TCM syndromes were significantly reduced,and the scores of TCM syndromes in treatment group was significantly lower than those in control group (P < 0.05).Conclusion In the treatment of stable COPD patients,Yupingfeng granule combined with Siwu granule can relieve inflammatory response and improve the lung function.
