保留旋前方肌掌侧微创入路手术治疗老年桡骨远端骨折疗效分析

摘　　要：目的探讨保留旋前方肌掌侧微创入路手术治疗老年桡骨远端骨折的有效性及其对腕关节功能的影响。方法选取2018年1月至2020年1月辉县市人民医院收治的64例老年桡骨远端骨折患者作为研究对象,按照随机数表法将其随机分为治疗组和对照组,每组32例,治疗组采用保留旋前方肌掌侧微创入路手术治疗,对照组采用常规Henry掌侧入路手术治疗,对比两组患者骨折愈合情况、腕关节主动活动度及腕关节功能。结果术后3个月,两组患者骨折部位均完全愈合,治疗组患者腕关节主动活动度优者19例、良者7例、中者4例、差者2例,优良率为81.25%,明显优于对照组的腕关节主动活动度优者8例、良者12例、中者9例、差者3例,优良率62.50%(Z=-2.548,P=0.011);治疗组患者Mayo评分为(74.25±6.47)分,显著高于对照组患者的Mayo评分(65.82±6.57)分(t=5.254,P<0.001)。结论保留旋前方肌掌侧微创入路手术治疗老年桡骨远端骨折,患者腕关节主动活动度及腕关节功能恢复效果更佳,临床效果更显著。Objective To investigate the effectiveness of the minimally invasive approach with preservation of palm side of pronator quadratus in the treatment of distal radius fracture in elderly patients,and its impact on wrist joint function.Methods Sixty-four elderly patients with distal radius fracture,admitted to Huixian People’s Hospital from January 2018 to January 2020,were selected as research subjects and then divided,according to the random number table,into a treatment group and a control group,with 32 cases in each group.Patients in the treatment group were treated with the minimally invasive approach with preservation of palm side of pronator quadratus,whereas patients in the control group with the conventional Henry approach.The fracture healing,active range of motion of wrist and wrist joint function were compared between the two groups.Results Three months after surgery,the fractures of patients in the two groups were all healed completely.In the treatment group,there were 19 cases with excellent active range of motion of wrist,7 cases good,4 cases fair,and 2 cases poor,and the excellent and good rate was 81.25%,being much better than 62.50%in the control group(8 cases excellent,12 cases good,9 cases fair,3 cases poor)(Z=-2.548,P=0.011).Mayo score in the treatment group was(74.25±6.47)points,much higher than(65.82±6.57)points in the control group(t=5.254,P<0.001).Conclusion In the treatment of distal radius fracture in the elderly,the minimally invasive approach with preservation of palm side of pronator quadratus can realize better recovery effects in terms of active range of motion of wrist and wrist function.
