摘　　要：目的:观察转化生长因子-β1(TGF-β1)和血管内皮生长因子(VEGF)因子的表达变化,探讨紫草油促进术后创面愈合的机制。方法:收集福建中医药大学附属人民医院肛肠二科2018年1月至2019年1月住院的120例肛裂术后患者资料,采用简单随机法将其分为紫草油组、凡士林组和湿润烧伤膏组,每组各40例。于肛裂术后第1天起至创面修复,分别给予紫草油、湿润烧伤膏、凡士林外用并统计3组的创面愈合时间。3组分别于第1、7、14、21天取材,利用实时荧光定量聚合酶链反应(Real-time PCR)和蛋白质印迹法(Western blot)检测3组血清中TGF-β1和VEGF mRNA和蛋白表达水平的变化。各组间比较采用F检验。结果:创面愈合时间结果表明,紫草油组愈合时间(19.20±2.86)d优于湿润烧伤膏组(21.58±3.88)d及凡士林组(26.55±3.64)d,差异有统计学意义(χ^(2)=53.109,P<0.05);Real-time PCR结果显示,术后第21天紫草油组中的TGF-β1和VEGF因子的mRNA分别为(1.934±0.030)ng/L及(2.001±0.028)ng/L,均高于凡士林组及湿润烧伤膏组,差异有统计学意义(F=479.094、937.85,P<0.05);Western blot结果显示,紫草油组的中的TGF-β1和VEGF的蛋白的表达量分别为(0.869±0.019)ng/L及(0.879±0.017)ng/L均高于其他两组,差异有统计学意义(F=13.699、20.127,P<0.05)。结论:紫草油能够促进术后肛裂创面愈合。对肛裂术后有良好的治疗作用,且疗效优于凡士林、湿润烧伤膏。Objective To study the change in transforming growth factor-β1(TGF-β1)and vascular endothelial growth factor(VEGF)expression,understanding mechanisms of lithospermum oil promoting wound healing.Methods Totally,120 cases of mixed hemorrhoids patients were divided into lithospermum oil group,vaseline group and moist burn ointment group by using simple random method,40 cases in each group.From the first day after anal fissure to the wound repair,lithospermum oil,moist burn ointment,and vaseline were applied separately and the wound healing time of the three groups were recorded.The samples were taken in three groups on the first day,the seventh day,the fourteenth day,and the twenty-first day.Real-time fluorescent quantitative polymerase chain reaction(Real-time PCR)and Western blotting were used to detect the changes of TGF-β1 and VEGF mRNA and protein expression levels in the three groups.F test was used for comparison between groups.Results Wound healing time in the comfrey oil group[(19.20±2.86)d]was significantly shorted than that in the moist burn ointment group[(21.58±3.88)d]and the petrolatum group[(26.55±3.64)d](χ^(2)=53.109,P<0.05).The Real-time PCR results showed that the mRNA levels of TGF-β1 and VEGF in the lithospermum oil group were(1.934±0.030)ng/L and(2.001±0.028)ng/L respectively on the 21st day after surgery,significantly higher than those in the vaseline group and moist burn ointment group(F=479.094 and 937.85,P<0.05).Western blotting results showed that the expression levels of TGF-β1 and VEGF protein in the lithospermum oil group were(0.869±0.019)ng/L and(0.879±0.017)ng/L respectively,significantly higher than those in the rest two groups(F=13.699,20.127,P<0.05).Conclusion Lithospermum oil can promote the wound healing,and its curative effectiveness is more satisfactory than vaseline and moist burn ointment in the anal fissure.
