摘　　要：目的分析慢性阻塞性肺疾病急性加重期患者(AECOPD)糖皮质激素受体基因D641V、R477H位点的多态性,并综合文献分析糖皮质激素受体基因多态性可能对疗效的影响。方法选取2017-2018年四川省内江市第一人民医院收治的AECOPD患者81例,外周血检测糖皮质激素受体基因D641V、R477H位点的单核苷酸多态性。结果 AECOPD患者D641V位点的检测:AA型81例,AT型0例,TT型0例;R477H位点的检测:GG型81例,GA型0例,AA型0例。结论 AECOPD患者糖皮质激素受体基因D641V、R477H突变率可能极低。综合文献分析,D641V、R477H突变者可能产生对糖皮质激素不同的敏感性。Objective To analyze the polymorphism of glucocorticoid receptor gene D641 V and R477 H in patients with AECOPD,and to analyze the effect of polymorphism of glucocorticoid receptor gene on curative effect. Methods 81 cases of AECOPD patients admitted to the First People’s Hospital of Neijiang,Sichuan Province from 2017 to 2018 were selected,the single nucleotide polymorphism of glucocorticoid receptor gene D641 V,R477 H locus was detected in peripheral blood.Results Detection of D641 V locus in patients with AECOPD: 81 cases of AA type,0 case of AT type,and 0 case of TT type;Detection of R477 H locus: 81 cases of GG type,0 case of GA type,and 0 case of AA type. Conclusion AECOPD patients with glucocorticoid receptor genes D641 V,R477 H mutation rate may be extremely low. Comprehensive literature analysis shows that D641 V,R477 H mutants may produce different sensitivity to glucocorticoids.
