摘  要:目的分析江苏省2015—2019年腹泻病人中沙门菌的耐药状况,为指导临床用药提供科学依据。方法生化鉴定和血清分型按照GB 4789.4-2016《食品安全国家标准食品微生物检验》沙门菌进行检测,按照美国临床实验室标准化协会(Clinical and Laboratory Standards Institute,CLSI)的操作规范推荐的肉汤稀释法进行药敏试验。结果共鉴定出36个血清型,其中肠炎沙门菌和鼠伤寒沙门菌是最主要的两种血清型。在1499株细菌中,对氨苄西林的耐药的菌株最多,达到69.1%。对四环素、萘啶酸、氨苄西林/舒巴坦、头孢唑啉、氯霉素、甲氧苄氨嘧啶/磺胺甲噁唑、头孢噻肟、环丙沙星、头孢他啶、阿奇霉素、庆大霉素、头孢西丁、亚胺硫霉素的耐药率分别为53.8%、50.6%、49.7%、43.5%、30.2%、29.7%、17.8%、17.0%、15.7%、14.9%、12.5%、8.1%、1.1%。所有的耐药菌株携带sulⅠ,tetA,tetB,blaTEM基因;94.0%的沙门菌携带tetG基因。结论江苏省食源性沙门菌对氨苄西林、四环素、萘啶酸、氨苄西林/舒巴坦普遍耐药,在临床用药时应选择对沙门菌敏感的药物。Objective To analyze antimicrobial resistance of Salmonella isolated from patients with diarrhea from 2015 to 2019 in Jiangsu Province and provide scientific evidence for clinical administration. Methods Biochemical identification and serotyping were performed according to GB4789.4-2016 Food Safety National Standard Food Microbiological Test for Salmonella, and the broth dilution method recommended by the Clinical and Laboratory Standards Institute(CLSI) of the United States was performed according to the operational specifications. The drug sensitivity test was performed. Results A total of 36 serotypes were identified, with Salmonella enterica and Salmonella typhimurium being the two most predominant serotypes. Among the 1 499 strains of bacteria, the most resistant strains to ampicillin were found, reaching 69.1%. The resistance rates to tetracycline, nalidixic acid, ampicillin/sulbactam, cefazolin, chloramphenicol, methomycin/sulfamethoxazole, cefotaxime, ciprofloxacin, ceftazidime, azithromycin, gentamicin, cefoxitin, and imethiomycin were 53.8%,50.6%, 49.7%, 43.5%, 30.2%, 29.7%, 17.8%, 17.0%, 17.0%, and 15.7%, 14.9%, 12.5%, 8.1%, and 1.1%, respectively. All drug-resistant strains carried sulI, tetA, tetB, blaTEM genes;94.0% of Salmonella carried tetG genes. Conclusion Salmonella foodborne in Jiangsu Province is generally resistant to ampicillin, tetracycline, nalidixic acid and ampicillin/sulbactam, and drugs sensitive to Salmonella should be selected for clinical use.
